Off pump bidirectional Glenn via right anterior thoracotomy

Abstract: The Glenn operation involving anastomosis of the superior vena cava to the pulmonary artery has been performed for palliative operations of many cyanotic congenital heart diseases in addition to the single ventricle since the 1960s. The classic procedure is done via median sternotomy and cardiopulmonary bypass. The benefits of this procedure without the use of cardiopulmonary bypass remain mixed within reported series. Cases using this approach and off-pump technique together in Latin-America have not yet been… Show more

“…Neste estudo, o tempo de hospitalização, tempo de extubação, incidência de complicações neurológicas e derrame pleural foram semelhantes entre os dois grupos. Outra análise mostrou ser possível realizar técnica de BG sem CEC com minitoracotomia direita, sem realizar a clássica esternomia (GUIDA M, et al, 2015).…”

“…In the study of Ulisses Alezandre Crotti, 7 the mean age of the on-pump group was 66 months and that of the off-pump group was 50 months ( p = 0.17 using the Mann–Whitney test). This result means that the differences in age, gender, weight and types of defects between the on-pump and the off-pump groups are not important factors in choosing the use of peripheral circulation.…”

“…The bidirectional cavopulmonary connection, also known as the ''bidirectional Glenn'' (BDG) anastomosis, a palliative operation performed in various congenital heart defects with 1 functional ventricle, is usually undertaken via sternotomy with cardiopulmonary bypass. Although there are a few reports of off-pump BDG via thoracotomy access, [1][2][3] to our knowledge a subaxillary approach has never been reported. We describe a case of bidirectional cavopulmonary anastomosis successfully performed through a small right middle subaxillary thoracotomy in an infant with tricuspid atresia in a developing country.…”
