Nutritional risk assessment in critically ill cancer patients: systematic review

Abstract: ObjectiveTo systematically review the main methods for nutritional risk assessment used in critically ill cancer patients and present the methods that better assess risks and predict relevant clinical outcomes in this group of patients, as well as to discuss the pros and cons of these methods according to the current literature.MethodsThe study consisted of a systematic review based on analysis of manuscripts retrieved from the PubMed, LILACS and SciELO databases by searching for the key words “nutritional ris… Show more

“…Albumin is an acute phase reactant that can be decreased in infections (Wierdak et al 2018). The association of albumin with the nutritional status is controversial and apparently varies among patient populations (Fuhrman et al 2004;Kuzuya et al 2007;Baron et al 2010;Gama-Axelsson et al 2012;Fruchtenicht et al 2015;Academy of Nutrition and Dietetics 2016;Bharadwaj et al 2016;Marcason 2017). Other laboratory parameters were also disturbed in SCAP patients (such as platelets, liver function, and natriuretic peptide), but whether they are causes or consequences of SCAP is unknown.…”

“…In the last 20 years, many screening instruments have been developed but most of these are too complicated for routine use in daily practices [13]. Until the present, no instruments have been devised that are sensitive and specific enough to become the golden standard for the assessment of nutrition [17]. Although there is no generally accepted method for assessing the nutritional status of patients, the assessment should establish the nutritional condition of a subject.…”

“…However, each parameter of the CONUT score has been reported to be associated with outcome in patients with GI and HPB malignancies. It is well recognized that the serum albumin plays an important role to reflect metabolic status as well as systematic inflammatory response in cancer patients [4,29]. Moreover, a low albumin level is not only associated with poor prognosis in various kinds of malignancies [6,25] but also found to be an independent predictor of postoperative complications in many studies [30][31][32].…”

“…Therefore, accurate nutritional evaluation of patients is very important. Although several nutritional assessment tools, such as Patient-Generated Subjective Global Assessment, nutritional risk index, malnutrition universal screening tool and the nutritional risk screening tool 2002 [2,3], have been developed, the precise evaluation including nutritional risk and outcome is still controversial in cancer patients [4].…”
