Introdução: 9,8% da população brasileira tem o hábito de fumar. Um estudo da OMS evidenciou que o tabagismo é responsável pela morte de mais de 8 milhões de pessoas por ano. No Brasil a dependência do tabaco causa aproximadamente 157 mil mortes ao ano, o que gera um custo anual para a sociedade de cerca de R$ 57 bilhões. O tabagismo é a principal causa de morte evitável no mundo. É uma condição de saúde epidêmica que causa dependência física, psicológica e comportamental. Estima-se que o tabagismo esteja relacionado a cerca de 50 doenças. Assim, esta pesquisa teve como objetivo levantar publicações que evidenciam a periculosidade que o uso do tabaco pode causar a saúde do ser humano. Revisão: Trata-se de uma revisão de literatura do tipo integrativa. O levantamento de dados científicos e a sistematização das informações são oriundas de produções científicas publicadas a partir de 2015, nos idiomas português e inglês, indexadas na base de dados da MEDLINE-PUBMED, LILACS e SCIELO. A nicotina afeta o sistema respiratório e causa diversos danos à saúde do fumante, tais como como doença pulmonar obstrutiva crônica, doenças intersticiais, exacerbações da asma, entre outras. O coronavírus pode representar uma ameaça adicional para os fumantes, sendo o tabagismo verificado como um fator de risco para admissão na UTI e mortalidade em pacientes internados tratados para COVID-19. Discussão: Embora a produção de tabaco movimente a economia, em contrapartida há um percentual muito maior com gastos em tratamentos da saúde. O uso de tabaco na juventude vem se alastrando e tornando os jovens mais propícios ao vicio proporcionando acometimentos a sua saúde. Considerações finais: O consumo do tabaco é extremamente prejudicial à saúde. Considerando o alto custo que esse hábito causa, campanhas massivas apoiadas pelo poder público, devem ser estimulas objetivando orientar, quanto aos riscos do uso desse hábito nocivo e evitável.

This study aimed to assess the subjective knowledge (SK) of Brazilian adolescent students about the health effects of smoking, and the association between the smoking status and their SK. A cross-sectional school-based survey was carried out. Participants were 3034 high-school students aged 13 to 19 from 13 municipalities in Midwestern Brazil. SK was assessed regarding three questions about the (1) general health effects of smoking, (2) oral health effects of smoking, and (3) health effects of secondhand smoking. For each question, SK was rated as low or high. Smoking status categories were (1) never smokers, (2) former smokers, and 3) current smokers. Poisson regression was used in the statistical analysis. Prevalence ratios (PR) and 95% confidence intervals (CI) were reported. About half of the participants had low SK about the general health effects of smoking, and most had low SK about oral health (62.9%) and second-hand effects (61.5%). Compared with smokers and former smokers, never smokers were more likely to have low SK about general (PR=1.52; 95%CI=1.25-1.85), oral (PR=1.17; 95%CI=1.03-1.32) and second-hand effects (PR=1.21; 95%CI=1.16-1.38) of smoking. In summary, adolescents tended to perceive that they had low knowledge about the effects of smoking and second-hand smoking on general and oral health. Low SK was associated with smoking status, meaning that, compared to smokers and former smokers, never smokers had the lower SK about the health effects of smoking.

Background The science of behaviour change has advanced considerably in understanding the psychological dimensions that make up individuals’ motivation to change their smoking behaviour. However, there is a lack of studies on the factors associated with the motivational system that prompts smoking initiation and cessation among adolescents. An important psychosocial factor related with smoking in adolescence, which has not yet been investigated in relation to their motivational system, is the Sense of Coherence (SOC). The present study aimed to estimate the extent to which the SOC is associated with the motivation to start and stop smoking among a population of adolescent students. Methods A cross-sectional school-based survey was carried out in Midwest Brazil. Participants were high-school students aged between 13 and 19 years who completed self-administered questionnaires. Explanatory variable was the SOC score, measured with Antonovsky’s Orientation to Life Questionnaire (13-item version). Motivation to start and stop smoking were measured with questions based on the Prime Theory of Motivation to address desire to smoke, belief on smoking benefit and intention to future smoking. Logistic regression was used in the statistical analysis, and a Directed Acyclic Graph was used to identify potential confounders. Results Participants (n = 2189) were 2073 never-smokers (94.7%) and 116 smokers (5.3%). The mean SOC score was 50.1 (95% CI = 49.6–50.7). About a quarter of the never-smokers were motivated to start smoking, and 14.7% of the smokers were not motivated to quit the habit. For each one-unit increase in the SOC score of never-smokers, the odds of desiring to smoke, believing on smoking benefit, and intending to smoke in the future decreased by 3%, 3% and 5%, respectively. Among smokers, the findings were inconclusive as to whether an association between SOC and motivation to stop smoking was present. Conclusion Increasing SOC was associated with decreasing motivation to start smoking among never-smokers. The findings indicate that SOC may play a protective role in the process of smoking initiation among adolescents, offering a potential adjuvant resource of theory-based interventions to prevent adolescent smoking.
