Introduction:The infant mortality rate (IMR) is an important health indicator directly associated with living conditions, prenatal care coverage, social development conditions, and parental education, among others. Worldwide, the infant mortality rate was 29/1000 live births in 2017. Therefore, this study aimed to evaluate the fetal and infant mortality rates due to congenital anomalies (CA) in Maranhão from 2001 to 2016. Methods: Data were obtained from the SINASC, and SIM databases. We used simple linear regression, Poisson distribution, and ANOVA (Bonferroni's post hoc test). We analyzed the public data (2001)(2002)(2003)(2004)(2005)(2006)(2007)(2008)(2009)(2010)(2011)(2012)(2013)(2014)(2015)(2016) of 1934858 births and determined the fetal, neonatal, perinatal, and post-neonatal mortality rates associated with CA by mesoregions. Results:The IMR in Maranhão was 17.01/1000 live births (95%CI, 13.30-20.72) and CA was the cause of death in 13.3% of these deaths. Mortality due to CA (per 1000 live births) was 0.76 (95%CI, 0.74-0.85) for fetal mortality rate and 2.27 (95%CI, 1.45-3.10) for infant mortality rate. Geographic and temporal variations were observed with a slight increase in recent years for deaths attributable to CA, and in the northern part of Maranhão. Conclusions:Mortality rates due to CA in Maranhão increased over the period 2001-2016 possibly as a result of improved maternal-infant health conditions eliminating other causes of death. Therefore, efforts to improve early diagnosis and better treatment of congenital anomalies should be considered to reduce its impact on child mortality.

O objetivo deste estudo foi avaliar o grau de implantação e de impacto do protocolo de vigilância do óbito por tuberculose (TB) em um município de grande porte e com elevada carga de TB. Para esta pesquisa avaliativa, elaborou-se um modelo lógico e matriz de julgamento. Os dados primários foram coletados a partir de um questionário aplicado aos profissionais e os dados secundários foram obtidos na Secretaria Municipal de Saúde de Manaus, Amazonas, Brasil. O impacto da adoção do protocolo foi analisado pelos indicadores de resultados e a evolução das taxas de incidência e mortalidade por TB, a partir da regressão linear simples e variação percentual. O grau de implantação da vigilância do óbito com menção de TB identificado foi parcial (51%). Quanto à evolução dos indicadores de incidência e mortalidade por TB, observou-se tendência significativa de aumento da taxa da incidência de TB no período de 2006 a 2019. A mortalidade por TB e a proporção dos óbitos com confirmação laboratorial para TB apresentaram incremento após a implantação do protocolo, indicando aumento da vigilância. A implantação parcial da vigilância do óbito de TB indica necessidades de mudanças no processo de trabalho da equipe que gerencia o Sistema de Informações sobre Mortalidade e o Programa de Controle da TB municipal. As mudanças observadas demonstram mais qualificação das informações do óbito por TB, assim como a vigilância no diagnóstico da TB contribuiu para a definição de casos com confirmação laboratorial para TB.

Objective: To compare 2012 and 2016 data on early neonatal near miss indicators from Health Information Systems at a university hospital. Methods: This is a cross-sectional study conducted in 2012 and 2016. We considered early neonatal near misses the live births that presented one of the following risk conditions at birth: gestational age <33 weeks, birth weight <1,750g or 5-minute Apgar score <7, or Neonatal Intensive Care Unit (NICU) admission, and were alive until the 7th day of life. Data were collected from the Live Birth Information System, Hospital Information System, and Mortality Information System. We calculated the early neonatal mortality rate, neonatal near miss rate, severe neonatal outcome rate, early neonatal survival index, and early neonatal mortality index, compared by year of birth. Results: In 2012, 304 early neonatal near misses were registered, with a higher proportion of cases with very low birth weight and mothers who had zero to three prenatal visits. In 2016, the number of cases was 243, with a predominance of more NICU admissions. The incidence of early neonatal deaths and early neonatal near misses was higher in 2012 than in 2016. Conclusions: Neonatal near miss indicators identified difference between years. The cases were more severe in 2012 and there were more NICU admissions in 2016.
