The scientific literature on the use of economic evaluation studies in health was reviewed to identify the costs related to hospital rehabilitation in public health services and/or systems. An integrative review was carried out on the Virtual Health Library (VHL) portal. The search strategy was built by the hubs: economic evaluation (phenomenon); hospital rehabilitation (population); and public health (context). Portuguese, Spanish and English were the limit of languages used. Two independent reviewers selected the publications using the PRISMA protocol. Data analysis was performed using the thematic mode. 11 articles were included. The following elements of the articles were summarized: country, method, objective, main results, type of economic evaluation, type of hospital rehabilitation and the context of public health systems or services. Finally, it focused on the boundaries between "cost analysis" and "economic evaluation" that studies used. The evidence gathered in this review allows us to conclude that the topic is very incipient, with a low level of scientific evidence available (levels 4 and 5). The studies range from cost analysis to economic evaluations. All studies considered as economic evaluation are cost-effective and converge in stating that hospital rehabilitation is cost-effective in relation to the diseases studied in the various health systems analyzed.

Objetivo: Descrever os custos das autorizações de internação hospitalar (AIH) por quedas de idosos no âmbito do Sistema Único de Saúde (SUS) do Brasil. Métodos: Estudo descritivo de custos, realizado a partir do Sistema de Informações Hospitalares do SUS, por meio das AIHs decorrentes de quedas de idosos (60 anos ou mais), no período de 2000 a 2020. Foram realizadas análises descritivas e mensurados os custos, por macrocusteio. Resultados: Foram registradas 1.746.097 AIHs por quedas de idosos, somando custos de R$ 2.315.395.702,75. A proporção de custos de internações foi maior entre aqueles com 80 anos ou mais (36,9%), mulheres (60,4%) e região Sudeste do país (57,3%). A média de permanência das internações variou de 5,2 a 7,5 dias. Conclusão: Os elevados custos identificados revelam a necessidade de investimentos em medidas mais efetivas, para prevenir e mitigar os danos decorrentes das quedas em idosos.

Cost-effectiveness of B-type natriuretic peptide-guided care in patients with heart failure: a systematic review

Measuring the level of B-type natriuretic peptide (BNP), as a guide to pharmacotherapy, can increase the survival of patients with heart failure. This study is aimed at systematically reviewing the studies conducted on the cost-effectiveness of BNP-guided care in patients with heart failure. Using the systematic review method, we reviewed the published studies on the cost-effectiveness of BNP-guided care in patients with heart failure during the years 2004 to 2017. The results showed that all studies clearly stated the time horizon of the study and included direct medical costs in their analysis. In addition, most of the studies used the Markov model. The quality-adjusted life years (QALYs) were the main outcome used for measuring the effectiveness. The studies reported various ranges of the incremental cost-effectiveness ratio (ICER); accordingly, the highest ratio was observed in the USA ($32,748) and the lowest ratio was observed in Canada ($6251). Although the results of the studies were different in terms of a number of aspects, such as the viewpoint of the study, the study horizons, and the costs of expenditure items, they reached similar results. Based on the results of the present study, it seems that the use of BNP or N-terminal pro-BNP (NT-pro-BNP) in patients with heart failure may reduce cost compared to the symptom-based clinical care and increase QALY. In this regard, these studies were designed and conducted in high-income countries; thus, the application of these results in low- and middle-income countries will be limited.
