To analyze the prevalence of anemia and associated factors in primiparous parturient. Cross-sectional study conducted in a municipality of the Brazilian Western Amazon from July 2014 to December 2015. A convenience sample of 461 first-time pregnant women were interviewed. Data on their sociodemographic, clinical, obstetric, personal habits and nutritional status were collected. Anemia and iron depletion were measured by peripheral blood collection with hemoglobin, hematocrit, serum ferritin and transferrin saturation index. To test the association between the variables, the χ 2 tests were applied and Poisson regression analysis with a 95% confidence interval was performed, and P < .05 values were considered significant. The Forward stepwise strategy was used to construct the adjusted model. These analyzes were performed using the STATA 14.0 program (College Station, TX, 2013). A higher risk of anemia was identified among adolescent; white; who had a partner; with unpaid occupation, with less than eight years of formal education. Residents in the countryside; smokers; who had more than six prenatal consultations and were overweight. Anemia was reported in 28.20% and iron depletion in 60.52% of parturient women. The variables studied did not have association with the anemia outcome, except alcohol consumption.

RESUMOIntrodução: A anemia é considerada um problema de saúde pública mundial, e segundo a OMS, acomete aproximadamente 24,8% da população. No Brasil, para combate da anemia ferropriva, foram implementados a fortificação obrigatória das farinhas de milho e trigo com ferro e ácido fólico e o Programa Nacional de Suplementação de Ferro (PNSF). Objetivo: Verificar através de uma revisão integrativa a efetividade da fortificação das farinhas e do PNSF. Método: Foram pesquisadas as bases PubMed, SciELO e LILACS e incluídos artigos originais publicados entre 2008-2017, sendo selecionados 14 artigos, 9 referentes à fortificação das farinhas e 5 referentes ao PNSF. Resultados: Enquanto alguns estudos demonstram adequação do estado do ferro e redução da prevalência de anemia após a fortificação, outros têm demonstrado uma falha em alcançar esses desfechos, principalmente gestantes, mulheres em idade reprodutiva e crianças com idade inferior a 24 meses. Baixa adesão ao PNSF pelas mães e profissionais da saúde, suplementação em quantidades inadequadas, problemas na operacionalização e fiscalização do programa, falta de treinamentos dos profissionais da saúde. Conclusão: Os resultados demonstram a necessidade de monitoração do programa de fortificação de farinhas a nível nacional, avaliando a quantidade de alimento fortificado consumida, o estado nutricional do ferro na população brasileira e a qualidade e biodisponibilidade do ferro dos alimentos fortificados. Os órgãos federais precisam se empenhar para melhorar a operacionalização do programa e incentivar a capacitação dos profissionais de saúde para que estes conscientizem a população sobre as consequências da anemia e sobre a necessidade de adesão ao PNSF.ABSTRACT Introduction: Anemia is a public health issue that affects nearly 24.8% of the world population, according to WHO. In order to reduce iron deficiency anemia prevalence, mandatory fortification of wheat and maize flour with iron and folic acid and National Program of Iron Supplementation (NPIS) were implemented in Brazil. Objectives: To verify the effectiveness of NPIS and flour fortification programs through a review. Methods: Pubmed, SciELO and LILACS databases were searched. Original articles published between 2008-2017 were selected, including 14 in total, being 9 related to flour fortification and 5 related to NPIS. Results: Some studies show the adequacy of iron status and reduction of anemia prevalence after flour fortification, while other did not have achieved the expected results, mainly in children under 24 months of age and pregnant and reproductive age women. A low adherence to the NPIS was observed for mother and health professionals, and there were problems in the distribution and monitoring of the program, as well as in the training of the health professionals.
