BackgroundStudies on telemedicine concentrate largely on children with congenital heart disease (CHD), in lieu of their respective parents. The psychological responses of parents of children with CHD may impact on their child's well‐being. This study aimed to examine the impact of telemedicine on the mental status of parents of children with CHD.MethodsA meta‐analytic approach was used to analyze scores on the Self‐reporting Anxiety Scale (SAS), the Self‐reporting Depression Scale (SDS), and the World Health Organization Quality of Life, Brief Report (WHOQOL‐BREF) of the parents of children with CHD. Scores were compared between the telemedicine group and the control group prior to and following the intervention using the measurement of mean difference (MD) with 95% confidence interval (CIs).ResultsA total of 209 studies were assessed, of which 10 were incorporated into the systematic review, while four other studies met the meta‐analysis inclusion criteria. Following the intervention, the SAS and SDS scores were significantly lower in the telemedicine group in comparison to the control group (p < 0.001). Telemedicine also exerted a substantial influence on lowering the parents' SAS (p < 0.001) and SDS scores (p < 0.001), as well as improving the quality of life of the parents (p < 0.001).ConclusionTelemedicine has a positive impact on lowering both anxiety and depression as well as improving the quality of life of parents of children with CHD.

Telemedicine in Surgical Care in Low‐ and Middle‐Income Countries: A Scoping Review

Background Access to timely and quality surgical care is limited in low- and middle-income countries (LMICs). Telemedicine, defined as the remote provision of health care using information, communication and telecommunication platforms have the potential to address some of the barriers to surgical care. However, synthesis of evidence on telemedicine use in surgical care in LMICs is lacking. Aim To describe the current state of evidence on the use and distribution of telemedicine for surgical care in LMICs. Methods This was a scoping review of published and relevant grey literature on telemedicine use for surgical care in LMICs, following the PRISMA extension for scoping reviews guideline. PubMed-Medline, Web of Science, Scopus and African Journals Online databases were searched using a comprehensive search strategy from 1 January 2010 to 28 February 2021. Results A total of 178 articles from 53 (38.7%) LMICs across 11 surgical specialties were included. The number of published articles increased from 2 in 2010 to 44 in 2020. The highest number of studies was from the World Health Organization Western Pacific region ( n = 73; 41.0%) and of these, most were from China ( n = 69; 94.5%). The most common telemedicine platforms used were telephone call ( n = 71, 39.9%), video chat ( n = 42, 23.6%) and WhatsApp/WeChat ( n = 31, 17.4%). Telemedicine was mostly used for post-operative follow-up ( n = 71, 39.9%), patient education ( n = 32, 18.0%), provider training ( n = 28, 15.7%) and provider-provider consultation ( n = 16, 9.0%). Less than a third ( n = 51, 29.1%) of the studies used a randomised controlled trial design, and only 23 (12.9%) reported effects on clinical outcomes. Conclusion Telemedicine use for surgical care is emerging in LMICs, especially for post-operative visits. Basic platforms such as telephone calls and 2-way texting were successfully used for post-operative follow-up and education. In addition, file sharing and video chatting options were added when a physical assessment was required. Telephone calls and 2-way texting platforms should be leveraged to reduce loss to follow-up of surgical patients in LMICs and their use for pre-operative visits should be further explored. Despite these telemedicine potentials, there remains an uneven adoption across several LMICs. Also, up to two-thirds of the studies were of low-to-moderate quality with only a few focusing on clinical effectiveness. Background: The number of Chinese clinical trials has continued to grow throughout the coronavirus disease 2019 pandemic, but we know little about clinical trial team members' perceptions and attitudes toward the impacts of the pandemic. This study aimed to assess the impact of the COVID-19 pandemic on clinical trials in China from the perspective of research staff to provide a deeper understanding and some recommendations for the ongoing and upcoming clinical trials during the pandemic.Methods: A nationwide cross-sectional questionnaire was distributed to respondents throughout mainland China between September 2021 and October 2021. The participants assessed the impact of the COVID-19 pandemic on clinical trials based on a 5-point Likert-type scale, and exploratory factor analysis (EFA) was used to confirm the factor structure. Descriptive statistical analysis and the Mann-Whitney test were used to discover the differences between different groups.Results: A total of 2,393 questionnaires from 272 hospitals were collected in mainland China. Factor analysis resulted in 4 factors, with a cumulative explained variance of 64.93%, as follows: subject enrollment, patient care, study supplies and data management, and research milestones and quality management. The research team members, predominantly represented by clinical research coordinators (CRCs), basically agreed with all but 3 preset scenarios of the impact of COVID-19 on clinical trials. Most respondents did not agree that the pandemic was associated with more serious adverse events (SAEs), missed reports of safety events, or any increase of unscheduled unblinding. In addition, significant differences were revealed in different age, gender, and role groups of respondents based on their views on the impact of the pandemic. Conclusions:The current pandemic situation has had a negative impact on clinical trials, especially in terms of subject recruitment and protocol compliance, yet research team members feel confident that some of the effective measures proposed in the study can moderate the negative impact.
