Background: Leprosy is a chronic infectious disease caused by Mycobacterium leprae. Patients have distinct clinical forms, and host´s immunological response regulate those manifestations. In this work, the presence of the myeloid-derived suppressor cell and the regulatory protein annexin A1 is described in patients with multibacillary leprosy and with type 1 and 2 reactions. Methods: Patients were submitted to skin biopsy for histopathological analysis to obtain bacilloscopic index. Immunofluorescence was used to detect myeloid-derived suppressor cells and annexin A1.Results: The data demonstrated that the presence of granulocytic and monocytic myeloid-derived suppressor cells in leprosy patients. The high number of monocytic myeloid-derived suppressor cells were observed in lepromatous leprosy and type 2 reactional patients with Bacillus Calmette–Guérin (BCG) vaccination scar. The presence of annexin A1 was observed in all myeloid-derived suppressor cells. In particularly, the monocytic myeloid-derived suppressor cell in the lepromatous patients has higher levels of this protein when compared to the reactional patients. This data suggest that the higher expression of this protein may be related to regulatory response against a severe infection, contributing to anergic response. In type 1 reactional patients, the expression of annexin A1 was reduced. Conclusions: Myeloid-derived suppressor cell are present in leprosy patients and annexin A1 might be regulated the host response against Mycobacterium leprae.

Background Although Ethiopia eliminated leprosy as public health problem 20 years ago, still more than 3000 new cases are reported annually. Leprosy related disability affects patients' day to day physical activities and their participation in social activities. Assessing the degree of activity limitation and social participation is recommended to show disability and assess the efficacy of rehabilitation efforts. Methodology and principal finding A hospital based cross sectional study was conducted among a total of 305 leprosy patients. Data were collected by face to face interview using Screening of Activity Limitation and Safety Awareness (SALSA) scale and participation scale. The analysis was done with SPSS version 25. Descriptive statistics was done and then binary logistic regression was used to identify factors associated with activity limitation as well as participation limitation. Most patients (219, 71.8%) had activity limitation; 41 (13.4%) with severe and 25 (8.2%) with extreme limitations. More than half of patients (168, 55.1%) were suffering from participation restriction; with 43 (14.1%) having severe restriction and 30 (9.8%) extreme restriction. Older age, low educational status, distance from treatment center, time of treatment and higher Eye, Hand, Foot disability score were associated with activity limitation. Similarly, older age, low educational status and being unmarried were significantly associated with participation restriction. Conclusion This study revealed that activity limitation and participation restriction are common among leprosy patients. Earlier diagnosis and improved rehabilitative services may help to decrease activity limitation, whereas community rehabilitation may improve social PLOS NEGLECTED TROPICAL DISEASES

Background: Hansen's disease is a disease that causes not only physical problem but also psychological and social problems. Too much emphasis on medical care can lead to neglect in the holistic rehabilitation of post-Hansen's disease persons. Aim: This study was carried out to assess the level of depression, quality of life and participation level of post-Hansen's disease persons living in SouthEast Nigeria. Methods: A cross-sectional survey was conducted among the post-Hansen's disease persons using the Major Depression Inventory for level of depression, SF-12 Health Survey for quality of life, and Participation Scale for participation level. The obtained data was summarized using descriptive statistics of frequency distribution tables and charts, mean and standard deviation, and analyzed using inferential statistics of Spearman's rank correlation and Chi-square test. Alpha level was set at 0.05. Results: A total of 99 persons (51 males, 48 females) within 22-88 years participated in this study. The results of this study showed that majority of the participants were severely depressed (79.8%), majority had poor quality of life (57.6%), and majority were moderately restricted in participation(56.6%).There was no significant correlation among the level of depression, quality of life and participation level. Age and gender did not significantly influence their level of depression, quality of life and participation level (p>0.05). Conclusion: The outcome of this study showed that majority of post-Hansen's disease persons living in SouthEast Nigeria are depressed with a poor quality of life.There is a need to implement measures that would improve the psychosocial and socioeconomic rehabilitation services for post-Hansen's disease persons.
