Background: The highest new case of leprosy in Indonesia was occupied by East Java Province for four consecutive years, with the number of new leprosy patients as many as 3880 in 2017. Purpose: This study aims to determine the characteristics of individual factors and risks factor for new leprosy cases. Methods: This research is observational analytic research with case control research design. The population of the case is new lepers that were diagnosed since January-September 2018. Meanwhile, the control population is skin poli patients who have not been diagnosed with leprosy and tuberculosis. Sampling technique used was simple random sampling. Variables studied were individual characteristics (age, gender, income) and individual risk factors including immunization status, contact history and population status. Results: Majority of the (57.90%) case respondents never received BCG immunization. 78.90% of the respondents case had history of contact with lepers before. 89.50% of respondents in cases and controls were permanent residents in Mojokerto Regency. Results of analysis of immunization status and contact history has significant influence to the new leprosy case. Status of not having BCG immunization (OR= 5.16; 95% CI= 1.23 <OR <21.55), p= 0.04) and history of having contact with lepers (OR= 6.43; 95% CI= 1, 52 <OR <27.24), p= 0.02) significantly affected new cases leprosy. Conclusion: There is an influence between non-immunization status and history of contact with new leprosy cases. It is important to carry out early detection of individuals in contact with lepers, especially household contacts and recommendations for repeated BCG vaccination

Objective: to analyze the trend of epidemiological indicators of leprosy related to diagnosis and physical disability. Methods: ecological study of time series. The secondary data were extracted from the Brazilian Notifiable Diseases Information System. Prais-Winsten generalized linear analysis was used for trend analysis. Results: 240,028 new cases of leprosy were analyzed. The Northeast region showed a decreasing annual trend (-2.9%) for general detection. As for disability, there are differences among the states of Bahia (4.9%), Alagoas (4.1%), Piauí (2.5%), Maranhão (2.2%) and Ceará (2.1%), which presented increasing trend to grade 2 ratio in the general population. Bahia (9.5%), Sergipe (6.6%) and Maranhão (4.9%) also presented an increasing trend to grade 2 among children. Conclusion: the Northeast region has remained at a very high endemicity level for leprosy with a difference in the distribution of the disease among the states. It was shown active transmission of the bacillus, late diagnosis and underreporting in the region.Objetivo: analisar a tendência dos indicadores epidemiológicos da hanseníase relacionados ao diagnóstico e incapacidade física. Métodos: estudo ecológico de séries temporais. Os dados secundários foram extraídos do Sistema de Informação de Agravos de Notificação brasileiro. Utilizou-se análise linear generalizada de Prais-Winsten para análise de tendência. Resultados: analisou-se 240.028 casos novos de hanseníase. O Nordeste apresenta tendência anual decrescente (-2,9%) para detecção geral. Quanto à incapacidade há diferenças entre os estados: Bahia (4,9%), Alagoas (4,1%), Piauí (2,5%), Maranhão (2,2%) e Ceará (2,1%) que apresentam tendência crescente para proporção de grau 2 na população geral. Bahia (9,5%), Sergipe (6,6%) e Maranhão (4,9%) também apresentam tendência crescente para grau 2 entre crianças. Conclusão: o Nordeste se mantém em nível de muito alta endemicidade para hanseníase com diferença na distribuição da doença entre os estados. Evidencia-se transmissão ativa do bacilo, diagnóstico tardio e subnotificação na região.

Abstract Papua and West Papua are two provinces in Indonesia with high rate of leprosy number. One of the problems in leprosy management is the emergence of leprosy reactions. The purpose of this study was to identified potential risk factors affecting to leprosy reaction among indigenous people in two leprosy pockets in Papua and West Papua. Cross-sectional studies were conducted in 10 months (February-November 2018) in Jayapura City and Bintuni District, by interview using questionnaire and physical examination. Successfully found 90 subjects of leprosy patients who are undergoing treatment and 18 subjects had experienced of leprosy reaction. Although no dominant potential risk factor was found, history of previous MDT consumption and history of BCG immunization become potential factors. Key words: reaction, leprosy, papua Abstrak Papua dan Papua Barat merupakan dua provinsi di Indonesia dengan angka kejadian kusta yang tinggi. Salah satu masalah dalam penanganan kusta adalah timbulnya reaksi kusta. Tujuan dari penelitian ini untuk mengidentifikasi faktor risiko potensial yang berpengaruh pada reaksi kusta pada etnis asli di dua kantong kusta di Papua dan Papua Barat. Penelitian potong lintang dilakukan selama 10 bulan (Februari-November 2018) di Kota Jayapura dan Bintuni, menggunakan wawancara dengan kuisioner dan pemeriksaan fisik. Berhasil ditemukan 90 subyek pasien kusta yang sedang menjalani pengobatan dan 18 di antaranya pernah mengalami reaksi kusta. Meskipun tidak ditemukan faktor risiko potensial yang dominan, namun riwayat minum MDT sebelumnya dan riwayat tidak diimunisasi BCG menjadi faktor-faktor yang potensial.
