Objective: construction and validation of a systematization instrument for Nursing Care, characterizing the profile of patients of an intensive care unit in the north of Brazil. Methods: it was a descriptive methodological study that followed a nursing process model in five phases. Results: it was suggested that the instrument be constructed in two formats; one for admission and another for daily assessment. Some items were removed from the instrument according to content validation content by the nurses. Of the 45 patients evaluated, 60.0% were men, 44.0% were married, 40.0% had low education levels, chronic renal failure and cancer. The main nursing diagnoses were risk for infection (100.0%) and impaired physical mobility (97.8%). The main nursing interventions were: handwashing, changing intravenous access, performing aseptic techniques and moving patients every 2 hours. Conclusion: construction and content validation were carried out successfully, promoting instruments capable of providing quality nursing care for patients in intensive care.

Este artigo objetivou refletir sobre a segurança do paciente em Unidade de Terapia Intensiva associada à Teoria das Necessidades Humanas Básicas de Wanda de Aguiar Horta, realizado em dezembro de 2015. Utilizou-se para o estudo a Cartilha dos 10 Passos para a Segurança do Paciente e as dimensões psicobiológicas, psicossociais e psicoespirituais da teoria. A escolha da Teoria se justifica pelo fato de seus elementos darem sentido ao cuidado de enfermagem em diferentes contextos como no ambiente crítico de cuidado. Resultou na elaboração de um quadro contendo a relação da Cartilha 10 Passos para Segurança do Paciente com a Teoria das Necessidades Humanas Básicas de Wanda Horta. Após a reflexão teórica, concluiu-se que existe conexão entre as necessidades básicas de Wanda Horta com os 10 Passos para a Segurança do Paciente, o que nos leva a inferir que a implementação adequada desta teoria sustenta um cuidado livre de riscos.

Objective: To understand the impact of soft tissue injuries in the provision of assistance to Basic Human Needs of customers by the nursing staff in the Adult Intensive Care Unit of a hospital in Pernambuco, Brazil.Methods: This is a descriptive study with quantitative character, performed with 104 nurses in December 2015. A questionnaire drawn from Wanda Horta's theory was used for data collection. Results:The most judicious care provided by the multidisciplinary team (81.7%) and the establishment of bond between professionals and clients (57.7%) were found to be the main positive effects. However, negative effects were outstanding, indicated by increased hospital stay (86.5%) and feeling of anxiety (72.1%). Despite the interference of lesions, body care (86%), communication (63.3%), and religiosity/ spirituality (43.3%) needs were referred to as met. Conclusion:The presence of wounds represents a difficulty to meet needs, although they bring some positive impact on the client. However, while recognizing the importance of valuing the customer subjectivity, this aspect is not yet addressed with proper attention.
