Objective: To identify the main challenges faced by children with type 1 diabetes mellitus and describe their coping strategies. Method: Qualitative study conducted with a semi-structured interview with children aged 8 to 11 with type 1 diabetes mellitus diagnosis who received care in the outpatient clinic of a university hospital in the state of Rio de Janeiro, Brazil. The results have been analyzed and categorized as per thematic analysis. Results: Five children have participated. The reports have emerged from the systematization of the qualitative data, in a process of apprehending meaning from statements of children and adolescents, which led to the category “Living with diabetes”, including two subcategories: “Challenges to disease coping” and “Family participation and support in the disease process”. Conclusion: The process of disease coping is observed to occur differently for each child. However, family participation and support, as well as communication with health professionals, are fundamental in this process.

Psychological Well-Being as an Independent Predictor of Exercise Capacity in Cardiac Rehabilitation Patients With Obesity

Objective: Exercise capacity (EC) is a well-established predictor of cardiovascular health. It is notoriously influenced by several factors, but the independent effect of psychological well-being (PWB) on EC has not yet been explored. The present study aims to investigate (1) whether PWB is an independent predictor of EC over and above selected demographic, behavioral, and biomedical parameters in a sample of CR patients with obesity and (2) whether PWB is a stronger predictor of EC than the other variables.Methods: Data from 1968 patients were collected at the time of their inclusion in a cardiac rehabilitation (CR) program and retrospectively analyzed in a cross-sectional study. Since cardiorespiratory parameters defined in normal weight populations differ from those of their obese counterparts, an ad hoc validated formula taking body mass index (BMI) into consideration was used to predict EC.Results: A multiple regression analysis revealed left ventricular eject fraction (LVEF) to be the strongest predictor of EC, followed by PWB, type 2 diabetes (DM), smoking status, atrial fibrillation (AF), and education. Bayesian evaluation of informative hypotheses corroborated LVEF as the best predictor of EC, and confirmed the superiority of PWB over and above DM and smoking status in influencing EC. Conclusion:These findings strengthen the link between psychological and physical health, suggesting a better PWB is associated with greater EC. Prompt screening of a patient's mood and readiness to perform an active lifestyle would therefore enhance the long-term health benefits of CR.

O Transtorno do Espectro Autista (TEA) é um transtorno do desenvolvimento neurológico, de maior prevalência no sexo masculino, caracterizado por dificuldades de comunicação e interação social e pela presença de comportamentos e/ou interesses repetitivos e restritos. As principais manifestações clínicas são: déficits persistentes na comunicação social e na interação social em múltiplos contextos e na presença de padrões restritivos e repetitivos de comportamento que causam prejuízos significativos no âmbito social e profissional. As evidências científicas comprovam que o diagnóstico precoce do TEA promove melhores prognósticos clínicos relacionados a intervenções clínicas e sociais de estimulação da neuroplasticidade neuronal das crianças diagnosticadas com TEA. Para isso, é essencial que o rastreamento pela Lista Modificada de Verificação do Autismo em Crianças (M-CHAT-R) seja um instrumento de rastreio amplamente utilizado no Sistema Público e Privado de Saúde em todo o território brasileiro. Muito além da sua aplicação, os profissionais de saúde necessitam de educação e instrução adequada para perceber os sinais de anormalidade das crianças sugestivos ao TEA, principalmente, os atrasos do desenvolvimento neuropsicomotor na faixa etária de 18 a 30 meses. O atendimento em saúde na Atenção Primária é essencial tanto no rastreamento, quanto no seguimento do tratamento após diagnóstico. Isso porque o vínculo terapêutico estabelecido entre equipe multiprofissional, família e paciente auxilia na melhora no quadro clínico, mas também dá suporte à família e à comunidade, na qual o indivíduo com TEA está inserido, de forma a garantir a sua emancipação social e econômica.
