Benign paroxysmal positional vertigo (BPPV) manifests itself as a paroxysm of vertigo and nystagmus lasting several seconds, which is self-limiting. The clinical characteristics and risk factors for the recurrence of BPPV in different ages have not yet been investigated.Materials and Methods: A retrospective observational study was conducted in the Department of Neurology in Beijing Tiantan Hospital from July 2009 to June 2015. The study included 1,012 patients aged 18–93 years. All patients received the definitive diagnosis and canalith repositioning maneuvers treatment and finally accomplished follow-up. Demographic variables, potential recurrence risk factors, neurological examination, and laboratory indexes were assessed.Data Analyses: The t-test or chi-squared test was first performed for group comparison, then logistic regression analysis was used to investigate the risk factors of BPPV recurrence.Results: The 1-year recurrence rates of BPPV patients after reposition maneuvers were, respectively, 22.79% (aged 18–45 years), 23.92% (aged 45–60 years), 28.89% (over 60 years). The recurrence rates among the three groups have no statistically significant difference. Logistic regression analysis shows that women BPPV patients have more recurrence risks than do men. Ménière's disease (odds ratio = 6.009, 95% confidence interval: 2.489–14.507, p < 0.001), hypertension (odds ratio = 1.510, 95% confidence interval: 1.095–2.084, p = 0.012), migraine (odds ratio = 2.534, 95% confidence interval: 1.164–5.516, p = 0.019), and hyperlipemia (odds ratio = 1.419, 95% confidence interval: 1.024–1.968, p = 0.036) were risk factors for the recurrence of BPPV in patients.Conclusion: We conclude that Ménière's disease, hypertension, migraine, and hyperlipemia may be independent risk factors for the recurrence of BPPV in patients, but aging does not increase the recurrence risk.

<p>A vertigem posicional paroxística benigna (VPPB), uma das causas mais frequentes de vertigem de origem periférica, é caracterizada por breves episódios de tontura desencadeados por movimentos cefálicos. Embora haja diversos tratamentos descritos para a VPPB, a manobra de reposicionamento otolítica é a mais utilizada, pela facilidade de execução e alta taxa de resolução dos sintomas. O objetivo deste estudo foi a avaliar os efeitos da manobra de reposição otolítica sobre os aspectos clínicos e funcionais dos pacientes com VPPB. Foi realizada uma revisão bibliográfica de artigos entre os anos 2000 e 2014, nas bases de dados virtuais SciELO, Medline e revistas eletrônicas. As palavras-chave usadas foram: vertigem, sistema vestibular, reabilitação, tontura e qualidade de vida, incluídos na pesquisa estudos de revisão, controlados, prospectivos, e que tinham no título ou no resumo o assunto inicial. Dos artigos encontrados, 37 foram escolhidos por atenderem aos objetivosdo estudo. Os aspectos funcionais descritos na VPPB foram o equilíbrio postural e o risco de quedas. Verificou-se que a manobra de reposição otolítica é eficaz e mantém os resultados por pelo menos três meses após a sua utilização, não necessitando de variantes ou associação com outras terapias para atingir um bom resultado terapêutico. A manobra de reposição otolítica tem boa efetividade com o restabelecimento do equilíbrio corporal em pessoas com VPPB. Restrições pós-manobra de reposicionamento não são necessárias para a resolutividade do quadro. Apesar disso, o acompanhamento clínico de longo prazo mostra que os idosos tratados apresentam prejuízo no equilíbrio corporal por perda funcional decorrente do processo de envelhecimento.</p>

Summary Introduction: Benign Paroxysmal Positional Vertigo (BPPV) is a very common vestibular disorder characterized by brief but intense attacks of rotatory vertigo triggered by simple rapid movement of the head. The integrity of the vestibular pathways can be assessed using tests such as digital vectoelectronystagmography (VENG) and vestibular evoked myogenic potentials (VEMP). Aim: This study aimed to determine the VEMP findings with respect to latency, amplitude, and waveform peak to peak and the results of the oculomotor and vestibular components of VENG in patients with BPPV. Method: Although this otoneurological condition is quite common, little is known of the associated VEMP and VENG changes, making it important to research and describe these results. Results: We examined the records of 4438 patients and selected 35 charts after applying the inclusion and exclusion criteria. Of these, 26 patients were women and 9 men. The average age at diagnosis was 52.7 years, and the most prevalent physiological cause, accounting for 97.3% of cases, was ductolithiasis. There was a statistically significant association between normal hearing and mild contralateral sensorineural hearing loss. The results of the oculomotor tests were within the normal reference ranges for all subjects. Patients with BPPV exhibited symmetrical function of the semicircular canals in their synergistic pairs (p < 0.001). The caloric test showed statistically normal responses from the lateral canals. The waveforms of all patients were adequate, but the VEMP results for the data-crossing maneuver with positive positioning showed a trend toward a relationship for the left ear Lp13. There was also a trend towards an association between normal reflexes in the caloric test and the inter-peak VEMP of the left ear. It can be concluded that although there are some differences between the average levels of the VENG and VEMP results, these differences were not statistically significant. Conclusion: In conclusion, the results of audiologic assessment, hearing thresholds, positioning maneuvers, and caloric tests have no effect on the quantitative results of VEMP. Additional research is warranted to establish the relationships among VENG, VEMP, and BPPV, especially as concerns the oculomotor tests.
