Total Arthroplasty in Ankylosed Knees: A Case Series

Abstract: OBJECTIVE: To present nine patients with ankylosis in their knees that were submitted to a total arthroplasty to lessen their pain and improve their functional limitation. For these patients, arthrodesis remained a possibility in the event of arthroplasty failure. INTRODUCTION: Ankylosis of the knee is a severe functional limitation that becomes worse when pain is present. Arthrodesis of the knee is a classical indication for such patients, since it resolves the pain; however, the severe functional limitation … Show more

“…Camanho reported good results with total knee arthroplasty in nine ankylosed knees followed up for at least 5 years, with 63° motion and a significant reduction in pain. 7 Two cases of late infection requiring surgical cleaning were reported. Naranja et al reported high complication and revision rates in patients who had arthroplasty for ankylosed knees.…”

“…Therefore, extensile approaches, including rectus snip exposure, V-Y quadricepsplasty, or tibial tuberosity osteotomy, have been needed in many cases. [4][5][6][7][8] These approaches were used for total knee arthroplasty with ankylosed knees by many surgeons, and several advantages and disadvantages were reported. 5,6,9,10 The advantages of the rectus snip are that it is simple and is closed in routine fashion, and there is no change in the standard postoperative rehabilitation protocol.…”

“…According to Comanho [7], who described 9 patients with ankylosed knee who underwent TKA, all showed improvement both in pain and ROM. Based on this observation investigators hypothesized that TKA is a treatment of choice in an ankylosed knee.…”

“…При анализе литературы, касающейся поставленной проблемы, был выявлен ряд статей зарубежных авторов, которые описывали выполнение операций тотального эндопротезирования с хорошим результатом даже у больных с костным анкилозом коленного сустава, т.е. в случаях, когда заведомо невозможно говорить о сохранной функции разгибательного аппарата [6][7][8]. Однако представленные научные исследования являлись клиническими наблюдениями или небольшими сериями наблюдений, что, безусловно, требует критического отношения к полученным результатам.…”
