Effect of heat treatment on stainless steel orthodontic wires

This study evaluated the effect of heat treatment on CrNi stainless steel orthodontic archwires. Half of forty archwires of each thickness -0.014" (0.35 mm), 0.016" (0.40 mm), 0.018" (0.45 mm) and 0.020" (0.50 mm) (totalling 160 archwires) -were subjected to heat treatment while the remainder were not. All of the archwires had their individual thickness measured in the anterior and posterior regions using AutoCad 2000 software before and after compressive and tensile strength testing. The data was statistically analysed utilising multivariance ANOVA at a 5% significance level. All archwires without heat treatment that were subjected to tensile strength testing presented with anterior opening, which was more accentuated in the 0.020" archwires. In the posterior region, the opening produced by the tensile force was more accentuated in the archwires without heat treatment. There was greater stability in the thermally treated archwires, especially those subjected to tensile strength testing, which indicates that the heat treatment of orthodontic archwires establishes a favourable and indispensable condition to preserve the intercanine width.

Introduction: In the context of rapid maxillary expansion, transverse deficiencies are the main predictors of dentofacial deformities with high prevalence in dental practice. In this sense, the techniques of surgically assisted rapid maxillary expansion (SARME) and non-surgically assisted rapid maxillary expansion (NSARME) are used in order to correct maxillary development through palatal disjunction. Objective: to analyze the main considerations of rapid maxillary expansion, showing its main indications, contraindications, types of expanders, as well as the main dental and skeletal changes produced by them. Methods: We searched the databases EBSCO, SCIELO, PubMed / MEDLINE and Academic Google in the period 2003 to 2018. The main descriptors (Mesh Terms) were Rapid expansion of the maxilla, Rapid assisted surgery of the maxilla, Rapid non-surgical maxillary expansion, Circuit breakers and Clinical studies. Main Finding: The use of the Hyrax expander has become a poorly indicated procedure since the vast majority of patients with maxillary atresia also have other vertical and / or sagittal discrepancies in the bony bases of the face. In this context, however, the rapid expansion of the surgically assisted maxilla presents as a safe and easy procedure to be performed in the office, and is also a low-cost and acceptable procedure for patients. Conclusion: Based on the literary findings, there is still no consensus on which is the best jaw breaker, and it is necessary to increase the number of clinical and randomized studies, with eligibility criteria that can in fact homogenize the participants. In addition, the literature has clearly shown that it is imperative that dental surgeons have the knowledge and clinical experience to be able to indicate the best procedure for each patient, that is, the rapid expansion of the non-surgical or surgical maxilla.

As deficiências transversais são deformidades dentofaciais relativamente comuns na clínica odontológica. Os pacientes nessas condições podem apresentar apinhamentos dentários, discrepâncias mandibulares, mobilidade dos dentes, perdas múltiplas dentárias, recessões gengivais, dificuldades respiratórias, sinusites de repetição, além de serem respiradores bucais. Nesse sentido, as técnicas de expansão rápida da maxila não cirúrgica (ERM) e cirúrgica são utilizadas, a fim de corrigir por meio da disjunção palatina o desenvolvimento maxilar. A ERM está contraindicada em pacientes com maturação óssea completa e, para esses casos, se indica a expansão rápida cirúrgica de maxila (ERMA), sendo esta preferível com expansor Hyrax. A técnica de expansão de maxila utilizando Hass mostra-se satisfatória devido à distribuição da tração ocorrer no centro da maxila e ser a técnica cirúrgica mais condizente com os parâmetros atuais e a menos invasiva com a utilização do expansor Hyrax considerando-se a higienização do paciente.
