Fatores de risco para a recidiva da tuberculose

Abstract: Recurrence of tuberculosis was more common in HIV-positive patients and in patients who did not comply with the self-administered treatment (RHZ regimen). Patients presenting at least one of these risk factors can benefit from the implementation of a post-treatment surveillance system for early detection of recurrence. An alternative to prevent noncompliance with tuberculosis treatment would be the use of supervised treatment.

“…Relapses were higher than the 4.3% found by Picon et al 17 in Rio Grande do Sul and lower than the 7.8% recorded by Yamagishi and Toyota 18 in Japan. Likewise, readmissions were lower than the 9.9% registered by Barbosa and Costa 19 in the city of Natal, but higher than the 3.6% described by Furlan et al 20 in the state of Paraná.…”

“…Relapses were associated in adults older than 40 years, similar to the findings of Oliveira and Moreira Filho 6 , but different from the findings of Picon et al 17 , who did not find association with age. This association of relapse with age can be explained as resulting from decreased immunity, concomitant diseases and malnutrition.…”

“…(7,11) Noncompliance with treatment for pulmonary tuberculosis in smear-positive patients favors the persistence of the transmission chain, as well as the increase in bacterial populations that are resistant to first-line chemotherapy. (12,13) This situation is a cause for concern, because it requires strategic measures to motivate patients to be more compliant with the treatment regimen.…”

“…Em um estudo que buscou identificar os fatores de risco para a recidiva da Tb, o uso irregular de medicamentos e a infecção pelo HIV mostraram-se relacionados com taxas maiores deste tipo de caso 14 . O predomínio do caso novo corrobora os achados da literatura.…”
