Rationale: Gastric trichobezoars are a rare form of bezoar formed from swallowed human hair as well as hair from dolls or animals, blankets, and carpets. They usually develop in young women who are emotionally disturbed, depressed, or mentally retarded, with trichotillomania and trichophagia. They can lead to abdominal pain, gastric ulceration, bleeding, obstruction, and perforation. Patient concerns: A 13-year-old girl was admitted to our institution with complaints of nausea, vomiting, and epigastric pain for 2 days. She underwent laparoscopic removal of a large gastric trichobezoar at our institution 5 years ago. A 12-year-old girl presented at our institution due to epigastric pain, dizziness, and melena for 2 days. Diagnosis: They were diagnosed with gastric trichobezoar by abdominal computed tomography scan and upper gastrointestinal endoscopy. Interventions: Large gastric trichobezoars were removed via a mini-laparotomy. Outcomes: They recovered well postoperatively without complication. Lessons: Although the recurrence of gastric trichobezoar after surgery is rare, few recurrent cases were reported in the literature. Therefore, psychiatric consultation and regular follow-up after treatment should be considered in the children and their parents to prevent the recurrence of gastric trichobezoar.

Introducción: Los bezoares son sustancias no digeribles que se acumulan en el tracto gastrointestinal, con mayor frecuencia en el estómago. El Tricobezoar gástrico (GT) es el bezoar más común que se encuentra en el estómago. Según su contenido, los bezoares se dividen en bezoares vegetales, bezoares capilares, bezoares de lactosa y bezoares medicinales. Las complicaciones más comunes reportadas a lo largo de los años incluyen erosión de la mucosa gástrica, ulceración y perforación gástrica o del intestino delgado, obstrucción de la salida gástrica, intususcepción, ictericia obstructiva, enteropatía por deficiencia de proteínas, pancreatitis y muerte. El tratamiento es la cirugía.Caso Clínico: Paciente femenina de 4 años presenta cuadro de tos y odinofagia, acompañado de dolor abdominal localizado en epigastrio, persiste que se exacerba con los movimientos, no se irradia, se acompaña con alza térmica no cuantificada, se realizan estudios de imagen en donde llama la atención eco abdominal con reporte de masa a nivel de epigastrio, se realiza radiografía de abdomen con evidencia de niveles. Realizan tomografía simple de abdomen en donde se confirma presencia de cuerpo extraño a nivel de estómago catalogado como bezoar.Evolución: Se le realiza laparotomía exploratoria + gastrostomía + extracción de tricobezoar + rafia gástrica, procedimiento sin complicaciones. Paciente estable, sin apoyo de oxígeno, no presenta signos de dificultad respiratoria, tolera alimentación, mejor control de dolor, con buena evolución postquirúrgica.Conclusiones: Los tricobezoar son extremadamente raros y plantean problemas de diagnóstico y tratamiento. Aunque actualmente la laparoscopia parece tener resultados prometedores en manos expertas, la laparotomía sigue siendo el tratamiento quirúrgico más exitoso. El tratamiento psicológico postoperatorio es fundamental para corregir las alteraciones psicológicas y conductuales de los pacientes y prevenir su recurrencia.
