Influence of assessment site in measuring transcutaneous bilirubin

Abstract: Objective:To investigate the influence of the site of measurement of transcutaneous bilirubin (forehead or sternum) in reproducibility of results as compared to plasma bilirubin. Methods:A cohort study including 58 term newborns with no hemolytic disease. Transcutaneous measurements were performed on the forehead (halfway between the headline and the glabella, from the left toward the right side, making consecutive determinations, one-centimeter apart) and the sternum (five measurements, from the suprasternal n… Show more

“…We speculate that the TcB measurement technique is not stable and consistent at the lower abdomen because abdomen is too soft in neonates, particularly tiny preterms. Forehead may be a more convenient site and better correlates with TSB, but skin over sternum may be typically light protected and therefore show greater correlation 8,9,10,11 . Recent systematic review of absolute TcB-TSB difference plots in preterms revealed minimal bias in measurement irrespective of site with pooled estimates of 0.83 12 .…”

“… 1 Recently published studies have shown that measurement of transcutaneous bilirubin on the sternum had higher accuracy as compared to serum bilirubin measurement on the forehead. 2 , 3 2) There are numerous different transcutaneous bilirubinometers (TcBm) in the clinical setting to estimate TcB. Comparative studies have shown varied agreement of TcBm with each other and TSB.…”

“…Even the studies which concluded that either site is superior, the difference is minute to have any clinical implications. He quoted 2 studies by Conceição etal 3 and Kosorat and Khuwuthyakorn. 2 Conceição et al 3 reported that the correlation between TSB and TcB as measured at the forehead had better correlation when compared with TcB measurement from the sternum (r=0.704 versus r=0.653).…”
