Three of those articles that described the pathophysiology and clinical presentation were published by nurses. One of the publications, using Roy's adaptation model, described the nursing process to clients with falciform anemia. Eleven publications (52.0%) were worldwide epidemiological studies. Seven publications (34.0%) described the diagnosis, neonatal screening, and programs for the management of falciform anemia in Brazil. Conclusion: There is a need for further research in the topic by health care professionals, especially by nurses regarding preventive measures, and the management and rehabilitation of patients with falciform anemia. Con relación a los descriptores, tres referencias (14%) son del área de enfermería, describen el cuadro clínico y la fisiopatología, siendo que una de ellas sistematiza la asistencia a la clientela con anemia falciforme bajo el marco teórico de adaptación de Roy; 11 (52%) destacaron estudios epidemiológicos y la distribución mundial; y siete (34%) contemplaron el diagnóstico médico, la clasificación neonatal y los programas dirigidos a la población con anemia falciforme, en Brasil. Conclusión: Los resultados apuntan la necesidad de realizar investigaciones en esa área por profesionales de la salud, principalmente los del área de enfermería, en relación a los cuidados de prevención, promoción y rehabilitación de los pacientes con anemia falciforme.

Methods An observational, cross-sectional study was conducted in the clinic of the Association of Persons with Hemoglobinopathies of Alagoas, in the city of Maceió-AL, Brazil. The sample composed of 43 patients were included by census sampling. The ICDAS/DMFT/dmf-t caries indices were used, and simplified oral hygiene, salivary flow and buffer capacity tests were performed. Patients were interviewed to check their level of knowledge about oral hygiene and diet. Spearman correlation and ANOVA statistical tests were applied (p<0.05). ResultsThe mean DMFT was 15.49±4.43; oral hygiene regular (IHOS=2.22±0.18); mean salivary flow 0.92±0.26, and mean buffer capacity 4.60±0.32. Correlation was found between salivary flow and buffer capacity (p = 0.009). Patients with the highest caries prevalence, in the age-ranges 6 to 12, and 20 to 26 years, had the lowest buffer capacity values (p = 0.04). Interviews showed that 58% did not use dental floss, 88% consumed cariogenic snacks, 100% reported never having received guidance from dentists about the cariogenic potential of foods and 60% reported the use of anticholinergic medications to reduce drooling (Tramal® and morphine) during pain crises. ConclusionThese patients presented extremely high caries prevalence, with predominance of carious teeth. The salivary parameters analyzed and level of knowledge about oral hygiene and cariogenic diet presented, showed that patients with sickle cell disease belonged to a group with risk for development of dental caries.

O Aconselhamento Genético (AG) constitui um processo de investigação do diagnóstico de doenças genéticas. Este estudo de caso objetivou a análise de um modelo de AG. A coleta de dados foi realizada por meio de entrevista semiestruturada e da observação de uma sessão de AG. Os participantes foram profissionais biomédicos e a consulente de uma criança com diagnóstico de síndrome do duplo Y. Verificou-se que o AG envolve uma relação intersubjetiva complexa. Percebeu-se, nos dois polos da relação – profissional e consulente-familiar –, os aspectos emocionais (angústia, temor, culpas etc.) e defensivos (identificação, racionalização etc.) referidos na literatura das crises vitais. A análise do processo de AG indicou: 1) dissonância entre a teoria do AG e a ação do profissional; 2) fixação (defensiva) ao protocolo do AG; 3) uso de linguagem técnica dificultando contato e acolhimento do consulente; e, 4) tempo de compreender considerado na dimensão cronológica e não lógico. Infere-se que os fatores afetivos (angústia, temor, culpas etc.) e a defesa psíquica (identificação, racionalização) restringiram a comunicação do diagnóstico comprometendo o acolhimento e esclarecimento do contexto vital do consulente. Conclui-se que o modelo de AG poderia se enriquecer com a experiência do modelo de aconselhamento psicológico no que se refere ao manejo de crises vitais e no trabalho em equipes multidisciplinares.
