AIDS-related lymphomas in Nigeria

Abstract: Aggressive non-Hodgkin's lymphoma (NHL), including primary central nervous system (CNS) lymphoma, lymphoblastic lymphoma and non-endemic Burkitt's lymphoma have been recognized as AIDS-defining cancers in most developed countries. However, HIV/AIDS epidemics appear not to have been associated with higher incidence of lymphomas in Africa. We therefore carried out this study to highlight the significance or otherwise of HIV/AIDS epidemics in the pathogenesis of lymphomas in a population of Nigerians with the dis… Show more

“…All those with a positive HIV status were found in the lymphoma group. This prevalence of HIV was higher than that reported in a study done in Ile-Ife, Southwest Nigeria among lymphoma patients in which 2.3% of the patients were HIV positive, 21 but lower than the reported 26.5% positive HIV status in another study from Cameroon. 22 The difference in HIV positive status may be explained by regional variation of HIV prevalence in the general population.…”

“…24 A study on AIDS-related lymphoma conducted in southwest Nigeria also documented majority of the patients presenting in advanced stages of the disease. 21 The reasons for late presentation of haematologic malignancies in our setting are multi-factorial, and ranges from patient factors, to delays in referral as well as paucity of adequate screening, diagnostic and therapeutic facilities. 10, 2527 …”

“…Although several studies suggest that the burden of NHL is increased among HIV positive persons in Africa [12,13,27,28], studies from West Africa have not supported this, just like our results. Similar to Bolarinwa et al , we identified several cases of NHL in the general population but none of these were matched in the HIV registry [18]. …”

“…However, the risk of non‐AIDS‐defining malignancies in an HIV‐infected population, including Hodgkin's lymphoma and some types of carcinoma , remains higher than that in the general population. Although CLL is a non‐AIDS‐defining malignancy and is common in Western countries, few reports have described cases of HIV concurrent with CLL in detail . Given that CLL patients are often immunocompromised and bendamustine has potent myelosuppressive activity, bendamustine treatment of CLL patients with concurrent HIV infection is associated with a higher risk of life‐threatening opportunistic infections.…”
