Primary amoebic meningoencephalitis: first reported case from Rohtak, North India

Abstract: A fatal case of primary amoebic encephalitis (PAM) in a 20 year old boy, a proven case of acute leukemic leukemia (ALL) type L2, in remission is described. No history of swimming could be elicited. The clinical presentation, the isolation of the amoeba from the cerebrospinal fluid (CSF), the poor response to amphotericin B, and the ultimately fatal outcome are all consistent with the diagnosis of PAM. On the basis of its ability to grow at temperature 42ºC and 45 ºC, morphology of trophozoite, and the presence… Show more

“…Naegleria usually transmitted while swimming or diving in contaminated water and has also been reported from other sources such as tap water and air [21, 22], rare cases have been reported where the infection occurred either by inhalation of air borne cysts or by face washing [23]. Among 7 pediatric reports of PAM, only two patients have had the history of contact.…”

“…Up to now, only Naegleria fowleri is known in humans as an etiological agent of PAM, which has a worldwide distribution of more than 300 reported cases, mostly from the United States [14]. It is almost always fatal; only three people in the United States out of 138 have survived infection [15].…”

“…Moreover, another simplified association using only 2 molecules, amphotericin B and rifampicin, named combination C PAM , has also been highlighted in the current work with higher efficiency compared with the combinations A PAM and B PAM : 38% for EDU value (Table 1). 59,[84][85][86][87][88][89][90][91][92][93][94][95][96][97][98] Therefore, the combination C PAM , which would be less toxic than the other combinations as it contains only two drugs, might also be recommended in PAM treatment, more probably at early stages of the pathology. Indeed, the celerity to give an accurate diagnosis during the pathology influences the complexity and the toxicity of the drug combination used.…”
