Objective: State outsourcing of the provision of health care policies is part of the neoliberal model. Early childhood policy in Medellin is managed under the public-private partnership modality with private non-profit institutions. The purpose of this study was to establish the philosophical foundations of the policy, the state’s control over the policy, pressure from interest groups and the working conditions of workers. Method: It is a qualitative study. Results: It was found that the philosophical foundations of the policy are public because the rights of the child inspire it; the state (city government) leads the policy, defines the guidelines and supervises compliance of objectives. However, the policy is carried out at the expense of the labor rights of professionals, and there is pressure on hiring by interest groups. Conclusion: The outsourcing of public policies to local non-profit organizations has adverse outcomes similar to policy outsourcing carried out through private companies.

Resumo Este estudo teve como principal objetivo analisar as políticas de recursos humanos em saúde nos hospitais estaduais gerenciados por organizações sociais de saúde no Espírito Santo. Para tal, efetuouse uma pesquisa qualitativa, elegendo-se como campo de investigação as organizações sociais que gerenciam hospitais públicos no estado. Na coleta de dados, buscaram-se as normativas e os contratos de gestão firmados entre 2008 e 2016 e realizaram-se entrevistas individuais com os gestores de recursos humanos das instituições participantes. Na análise dos materiais de campo, lançou-se mão da técnica de análise de conteúdo, sendo eleitas cinco categorias empíricas de análise dos contratos: modelo de vínculo empregatício, permissão de contratação de pessoa jurídica, percentual de gastos com pessoal, procedimento de contratação de pessoal, metas relativas à gestão de pessoas. As entrevistas foram analisadas pelas unidades de significação como proposto por Kvale. Os resultados demonstraram que os contratos omitem informações exigidas legalmente e possuem aditivos em demasia. As práticas de gestão de pessoas são próprias de empresas privadas, atendendo a algumas orientações do trabalho em saúde, contudo não referenciando o Sistema Único de Saúde. Adotam métodos divergentes das regras do concurso público e da não precarização do trabalho. Palavras-chave organizações sociais; recursos humanos em saúde; gestão hospitalar. AbstractThe main goal of the present study was to analyze the politics of human resources in health in state hospitals managed by health social organizations in the state of Espírito Santo, Brazil. We conducted a qualitative research, choosing as the field of investigation the social organizations that manage public hospitals in the state. During the data collection process, we sought the regulations and management contracts signed between 2008 and 2016, and performed individual interviews with the human resources managers of the participating institutions. During the analysis of the fieldwork materials, we used the content analysis technique, and chose five empirical categories of analysis of the contracts: the employment relationship model, the permission to hire legal entities, the percentage of expenses with staff, the procedure used to hire staff, and the goals regarding people management. The interviews were analyzed through the units of meaning proposed by Kvale. The results showed that the contracts omit legallyrequired information and have too many amendments. The people management practices are typical of private companies, and follow some guidelines of the work in health, but without reference to the Brazilian Unified Health System (Sistema Único de Saúde, SUS, in the Portuguese acronym). They adopt methods that are different from the rules of the public service entrance exams and the non-precarization of labor. Resumo No Brasil, a expressão 'gestão do trabalho' passou a ser utilizada após a 12ª Conferência Nacional de Saúde em substituição a 'recursos humanos'. O objetivo foi analisar, com base no contexto histórico brasileiro, os sentidos atribuídos à expressão 'gestão do trabalho em saúde' na literatura científica brasileira, por meio de scoping review na base Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs), entre 1990 e 2010. De 436 estudos identificados, 34 foram incluídos. Houve crescimento absoluto das publicações após a 3ª Conferência Nacional de Gestão do Trabalho e Educação em Saúde, 2006. O conteúdo foi sintetizado em cinco unidades temáticas: conceitos de gestão do trabalho em saúde, formulação de políticas, participação dos trabalhadores nos processos de trabalho, precarização do trabalho e novos desafios. Observou-se consenso sobre a complexidade da expressão e os aspectos distintos foram explorados dependendo das características políticas, sociais e econômicas no momento da construção do pensamento. As discussões teóricas deveriam estimular mudanças nas práticas de gestão e cultura dos modelos de produção, porém as mudanças são escassas, restando um questionamento não desvelado: gestão do trabalho em saúde, uma mudança terminológica ou de paradigma? AbstractThe expression 'work management in healthcare' began to be used in Brazil after the 12ª National Conference of Health, replacing 'human resource'. The aim was analyze the meanings attributed to the use of the term 'work management in healthcare' based on the Brazilian historical context. This research developed a scoping review in the Lilacs database, between 1990 and 2010. Among 436 eligible studies, 34 were included. These publications presented numerical growth especially after the 3 rd National Conference of Work Management and Education in Healthcare at 2006. The content was synthetized in five main topics: conceptualizing of work management in healthcare, policy formulation, and participation of workers in the work process, labor precarization and new challenges. Despite the consensus about the complexity of the expression, distinct aspects are explored by the authors depending of political, social and economic contexts during the construction of thought. The theoretical discussions should support changes on practices of management and on culture of production modeling, but practical changes are scarce, generating a question that was not completely answered: does work management in healthcare represent a simple terminology exchange or paradigm shift?
