Objective: To compare the oral health condition of children and adolescents victims of maltreatment housed in foster care with that of children and adolescents that have not been victims of maltreatment. Material and Methods: This is a case-control study with children and adolescents that have not been victims of maltreatment. In Group 1, the population was composed of 56 children victims of abuse housed in foster care. Group 2 was composed of an equal number of children and adolescents attending pediatric dentistry clinic at UFPI. Both groups were matched for sex and age. Data collection was divided into two phases: questionnaire application to children's parents / guardians and clinical examination of the oral cavity. The questionnaire was composed of questions about socioeconomic variables, reasons for admission in the foster care and oral healthrelated habits. To assess the oral health condition, epidemiological indexes dmft, DMFT and Gingival Bleeding Index (GBI) were determined and soft tissues were inspected to evaluate possible maltreatment sequelae. Results: Neglect was reported as the major cause of entry into the foster care (84%). There were no statistically significant differences between the mean dmft (p=0.240), DMFT (p=0.862) and GBI (p=0.275) values between groups evaluated. No sequelae or lesions characteristic of physical aggression were found; however, all individuals have been institutionalized for more than four months. Conclusion: Child and adolescents victims of maltreatment showed oral health condition similar to that of individuals that have not been victims of maltreatment.

“…The orofacial signs, if present, may appear ranging from erythema to ulceration and from vesicle-pustular to pseudomembranous lesions on the tongue, palate, cheek, and mainly on the pharynx, and HPV lesions, erythema and petechiae at the junction of the hard and soft palate or floor of the mouth, suggesting forced oral sex. There is another pathognomonic sign of sexual abuse that is the presence of oral and perioral gonorrhea in prepubertal children, which although not very often can be diagnosed by laboratory tests [20]. Introduction: Violence against minors can manifest in many forms and is considered a public health problem due to the physical and emotional consequences it produces. The dentist has a fundamental role in detecting victims of violence, as the face is often the most affected region. Objective: To review the national and international literature concerning injuries and most common oral conditions of children and adolescents victims of abuse. Literature review:The descriptors used were "child abuse", "child violence", "physical violence", "psychological violence", "dental caries", and "oral health" for international databases and the corresponding terms in Portuguese for Brazilian databases, with no restriction of year of publication and language. Through the articles included in this review, it appears that children with maltreatment history presents not only high prevalence of head injuries and dental traumas, but may also have poor oral hygiene and low search for dental services and consequently higher rates of untreated caries compared with children who lived in contexts without violence. Conclusion: Although the literature suggested that children and adolescents who have suffered abuse may have worse oral health status than those who were not victims of violence, more studies are needed to understand whether there are actually significant differences in the prevalence trauma, soft tissue injuries, tooth decay, and periodontal disease among types of child abuse.

ResumoEste estudo objetivou identificar tipos de maus-tratos mais prevalentes incorridos contra crianças e adolescentes, características indicativas de maus-tratos observadas durante atendimento odontológico, além de descrever a conduta do cirurgião-dentista frente a esses casos. Os trabalhos científicos foram selecionados a partir da Biblioteca Virtual de Saúde; 24 artigos atenderam aos critérios pré-definidos. Um manual sobre notificação de maus-tratos infantis do Ministério da Saúde Brasileiro, e texto da Lei 8.069 também foram revisados. Observou-se que maus-tratos contra crianças e adolescentes comumente se manifestam em negligência, abusos físicos, sexuais e psicológicos. Ademais, constatou-se que o cirurgião-dentista pode exercer importante papel na identificação de maus-tratos infanto-juvenis, já que a maioria das lesões físicas oriundas de maus-tratos envolvem cabeça, pescoço e boca. Com base no Estatuto da Criança e Adolescente, o cirurgião-dentista é obrigado a identificar e notificar casos suspeitos de maus-tratos, encaminhando-os para o Conselho Tutelar. No entanto, notou-se inabilidade de cirurgiõesdentistas com relação ao diagnóstico e notificação. A subnotificação de casos de maus-tratos dificulta o dimensionamento epidemiológico do problema e consequente desenvolvimento de programas e ações para seu enfrentamento. Acredita-se que inserir o assunto "maus-tratos infanto-juvenis" nos projetos pedagógicos dos cursos, bem como divulgar práticas de notificação e conduta, contribuirão para erradicação desses maus-tratos.Palavras-chave: Abuso infantil, Adolescentes; Cirurgião-dentista.
