OBJECTIVE Analyzing statements of health professionals from a Street Clinic regarding care of a homeless population with tuberculosis. METHOD This is a qualitative research, conducted in the central region of São Paulo at three basic health units in the period of November to December 2014. A semi-structured interview guideline was implemented for data collection and all interviews were recorded using a digital recorder. RESULTS Six health professionals were interviewed. According to the Discourse Analysis perspective, three discursive segments emerged: experiences on care in the streets; weaknesses inherent to the treatment process; and incentives as a means of maintaining sick people in treatment. CONCLUSION Caring for a the homeless population with tuberculosis constitutes a new and challenging experience. It involves difficulties in dealing with the reality of a miserable social context, a lack and inadequacy of services, as well as care limitations for treatment and treatment dropout, which reinforces multiresistance. However, the investigated Street Clinic teams seek to expand access to health and social care services to this population.

Introduction: Tuberculosis (TB) remains as a major public health problem, being the second infectious disease to cause death all over the world. The Directly Observed Treatment (DOT) is one of the recommended strategies to control this disease. The accession in this mode of treatment is integrated into the Tuberculosis Control Program (PCT), this program also provide socio-economic incentives as the distribution of food baskets, bus passes and breakfast. Moreover, public policies for social protection help both family development and social vulnerability. Tuberculosis is still associated with social exclusion and marginalization of the population subjected to poor living conditions and limited access to public services.Objective: Analyze the potential accession to tuberculosis treatment related to individual and social vulnerability aspects. Methods:Cross-sectional, qualitative study, conducted with 39 individuals diagnosed with tuberculosis and in treatment for at least 30 days in the city of Campina Grande, Paraiba, Brazil. 20 markers were used as analysis units, with the possibility of score responses from one to three. The lower scores indicate lower potential of adherence to treatment, while the highest ones express greater potential accession.Results: xx markers that were more related to score one and indicated lower potential adherence to tuberculosis treatment were: tuberculosis impact on labor; conception of health-disease causality and work process (employment status). Resumo: O artigo apresenta resultados da investigação sobre as estratégias de interpretação pelas quais campanhas publicitárias delimitam um lugar discursivo ao sujeito-preventivo das doenças tropicais transmitidas pelo Aedes aegypti. Para isto, mobiliza conceitos da Análise de Discurso francesa, principalmente os conceitos de formações imaginárias, ideologia e posição-sujeito. Filiado ao paradigma indiciário de análise, o estudo analisa cinco imagens e enunciados veiculados nestas campanhas e mostra que o lugar do sujeito-preventivo é marcado pela culpabilidade individual, por meio de uma lógica disjuntiva sobre a responsabilidade tanto ao espaço privado quanto à esfera pública que ignora a conjuntura sociopolítica mais complexa de seu entorno. Palavras-chave: doenças tropicais; Aedes aegypti; promoção da saúde; saúde pública; campanha.
