Dentin permeability: the basis for understanding pulp reactions and adhesive technology

Abstract: Permeability involves the passage of fluids, ions, molecules, particulate matter and bacteria into and through a substance or tissue under different and varying conditions. The permeability of the dentin is essential to support the physiology and reaction patterns of the pulp-dentin organ. Nutrients and impulses are transported from the pulp via the odontoblast process and the contents of its tubules maintain the dentin as a vital tissue. However, the main interest of this paper focuses on penetration from the… Show more

“…Dentin that is devoid of a smear layer is highly sensitive because products and stimuli are easily disseminated to the pulp. 2,14,16 Furthermore, an incompatibility between the gel and the adhesive The same letter indicates a lack of significant difference (p < 0.05). The same letter indicates a lack of significant difference.…”

“…1 CDH is characterized by an acute and short-term pain, which can be triggered by the touch of an instrument, brush bristles, and cold or sweet food and drinks. 2 The population prevalence of CDH ranges from 10% to 20% at age 40, and it affects mainly canines and premolars. 1,3 To date, no method has been developed to resolve CDH satisfactorily, and recurrences are common.…”

“…Second, the presence of mastication forces in clinical situations and the presence of positive intrapulpal pressure and continual outflow of fluids and plasma in dentinal tubules, which have a dramatic effect on cement adhesion to tooth structure and the microleakage score, were not simulated in this experimental setup. 29 Furthermore, the dimensions of methylene blue were extremely small compared with those of a typical bacterium (500-100 nm). Therefore, the microleakage scores of in vitro dye penetration studies may not be clinically relevant.…”

“…70,71 Dentin yüzeyine asit uygulanması, intertübüler dentin matrisini çevreleyen kollajen fibrilleri açığa çıkararak adezivin demineralize matris içerisine sızmasına ve hibridizasyonun sağlanmasına imkan verse de bu işlem hassas olmayan dentin kanallarını da ortama açabilmekte, post operatif hassasiyete neden olarak tedaviyi güçleştire-bilmektedir. 71,72 Kendinden asitli dentin adeziv sistemlerin dentin yüzeyine asit uygulanmasını gerektirmemesi ve yüzeyde bulunan smear tabakasını modifiye ederek oluşacak hibrit tabaka sayesinde de dentin hassasiyetinin önlenmesin-de etkili olabilecekleri bildirilmiştir. [73][74][75] Baysan ve Lynch 73 19 ay boyunca test ettikleri kendinden asitli özellikteki hassasiyet giderici ajanın etkili bir şekilde hassasiyeti önlediğini ve 19 ay sonunda dentin yüzeylerinde hassasiyet giderici ajanın halen mevcut bulunduğunu saptamışlar-dır.…”
