Simplified laparoscopic cholecystectomy with two incisions

Abstract: BackgroundLaparoscopic cholecystectomy has traditionally been performed with four incisions to insert four trocars, in a simple, efficient and safe way.AimTo describe a simplified technique of laparoscopic cholecystectomy with two incisions, using basic conventional instrumental.TechniqueIn one incision in the umbilicus are applied two trocars and in epigastrium one more. The use of two trocars on the same incision, working in "x" does not hinder the procedure and does not require special instruments.Conclusio… Show more

“…Laparoscopic cholecystectomy has become the gold standard for surgical treatment of benign gallbladder disease. The advantages of this procedure compared to laparotomy include smaller incisions, reduced surgical trauma, less postoperative pain, faster recovery times, a shorter hospital stay, and faster return to normal activities and work 1 , 7 , 25 , 26 . The use of a single umbilical incision for gallbladder removal was an interesting innovation and single-incision laparoscopic cholecystectomy has gained momentum in the literature since its description by Navarra et al 21 .…”

“…From the aesthetic point of view, the marionette technique is superior to the conventional one, since this involves only two scars (umbilical and epigastric, Fig. 5) with the advantage of avoiding additional two incisions [20]. In some study, cosmesis considered the main outcome; however, there was evidence to support that, the inherent risks of this surgery are increased potentially by the technical difficulty associated with the 1-port technique [21].…”

“…Several surgical options for TILC have been reported. [13][14][15][16] However, some options require special instruments during the operation, such as a 2-mm endoscopic grasper and a 000 monofilament nylon suture with a straight needle, which increase the costs to patients. Other options involve difficulty exposing the surgical visual field caused by inappropriate incision position.…”
