Objective: Cystic fibrosis (CF) is a chronic multisystemic hereditary disease for which a multidisciplinary approach must be taken. The objective of this study was to show the evolution of a group of patients with CF after the implementation of multidisciplinary treatment. Methods: A retrospective study involving 19 patients (6-29 years of age) under clinical follow-up treatment at the University of São Paulo at Ribeirão Preto School of Medicine Hospital das Clínicas, located in the city of Ribeirão Preto, Brazil. The patients were divided into two groups:6-12 (6-12 years of age) and 13+ (> 12 years of age). We collected data regarding body mass index (BMI), Z score, Shwachman score (SS), number of exacerbations/year, chronic colonization by Pseudomonas aeruginosa, and spirometric measurements (FVC, FEV 1 , FEV 1 %, and FEF 25-75% ). Data were collected at two different time points (before and after the implementation of the multidisciplinary treatment) and were analyzed with the Wilcoxon signed rank test. Results: The median age at the onset of symptoms was 10 months. In the 6-12 group, only BMI and FVC increased significantly. Although the other spirometric values increased, the differences were not significant. In the 13+ group, there were no significant differences between the two time points. There was a borderline significant decrease in SS and less than significant decreases in the spirometric measurements. However, the number of patients with alterations in volumes and flows decreased in both groups. Conclusions: Although our patient sample was small, the lack of changes in the spirometric parameters might reflect clinical and functional stability. In all of the patients evaluated, clinical, functional, and nutritional parameters remained stable throughout the study period. The implementation of a multidisciplinary approach might have contributed to this result.Keywords: Cystic fibrosis; Spirometry; Body mass index; Physical therapy modalities. O objetivo deste estudo foi mostrar a evolução de um grupo de pacientes com FC após a implantação desse tipo de atendimento. Métodos: Foram analisados retrospectivamente 19 pacientes (idades entre 6 e 29 anos) em acompanhamento clínico no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto da Universidade de São Paulo, na cidade de Ribeirão Preto (SP). Percepção da gravidade da doença em pacientes adultos com fibrose cística

OBJETIVO: Avaliar a percepção da gravidade da doença em pacientes com fibrose cística (FC), investigando sua relação com escore clínico, escore radiológico, testes de função pulmonar, adesão ao tratamento e percepção de autocuidado. MÉTODOS: Estudo transversal, prospectivo, envolvendo pacientes com FC atendidos em um programa para adultos com FC. A percepção da gravidade da doença, a adesão ao tratamento e o relato de autocuidado foram avaliados por questionários. Foram obtidos de todos os pacientes dados clínicos, escore clínico de Shwachman-Kulczycki, escore radiológico de Brasfield e espirometria. RESULTADOS: De 38 pacientes estudados, 3 (7,9%) relataram percepção de sua saúde como muito abaixo da média; 5 (13,2%), como abaixo da média; 15 (39,5%), como na média; 10 (26.3%), como acima da média; e 5 (13,2%), como muito acima da média. A percepção da gravidade da doença correlacionou-se significativamente com o escore clínico (r = 0,43, p = 0,007), CVF (r = 0,34, p = 0,034), VEF1 (r = 0,38, p = 0,019) e com relato de autocuidado (r = 0,33, p = 0,044), mas não com o grau de adesão (r = -0,03, p = 0,842) e escore radiológico (r = 0,33, p = 0,51). CONCLUSÕES: A percepção da gravidade da doença se relacionou com medidas objetivas de gravidade da doença (escore clínico e testes de função pulmonar) e com relato de autocuidado, mas não com a adesão ao tratamento.

_________________________________________________________________________________ RESUMOIntrodução: A Fibrose Cística (FC) é uma doença autossômica recessiva letal, que acomete preferencialmente crianças brancas. Objetivo: Descrever e discutir os mecanismos moleculares envolvidos na FC. Materiais e métodos: Artigos obtidos nos bancos de dados internacionais SCIELO, PUBMED, BIOLINE, DOAJ e HIGHWIRE que tratam dos mecanismos moleculares envolvidos na FC. Resultados e discussões: Decorre de mutações no gene que codifica a proteína "regulador de condutância transmembrana da fibrose cística" (CFTR). Esta contribui para a regulação do fluxo iônico nas superfícies apicais das células epiteliais, e é um canal de cloreto. A fisiopatologia da FC envolve o bloqueio da secreção do cloreto através da membrana apical das células epiteliais glandulares, resultando em seu acúmulo intracelular, acompanhado do influxo de sódio e água para este compartimento. Como consequência, ocorre desidratação da superfície celular com formação de secreções espessas -características da doença. O diagnóstico é baseado na clínica, demonstração laboratorial de concentrações elevadas de cloreto no suor e análise genética. O tratamento inclui: fluidificação das vias aéreas, antibioticoterapia, broncodilatadores, suporte nutricional e reposição enzimas. Na tentativa de aumentar o efluxo epitelial de Cl -ou inibir o influxo do Na + , alvos moleculares podem ser atacados por diferentes drogas como reguladores de migração citoplasmática, ativadores da expressão gênica, entre outros, para o primeiro, e inativação dos ENaC (amilorida ou benzamil) ou inibição do influxo de sódio (ouabaína, digoxina ou loperamida). Considerações Finais: Novas modalidades terapêuticas e a detecção precoce da FC por genotipagem podem melhorar o prognóstico e a qualidade de vida dos afetados. As consequence will occur a dehidratation of the cellular surface with thick secretion.
