OBJECTIVE:The International Classifi cation of Primary Care was developed as an attempt to overcome the limitations of the International Statistical Classifi cation of Diseases and Related Health Problems, 10th revision, when used for primary health care. The aim of the study was to evaluate the interobserver reliability of the International Classifi cation for Primary Care when coding reasons for health-related interruption of daily activities. METHODS:Data analyzed pertained to 801 subjects from Phase 2 of the Pró-Saúde Study, involving the employees of a Rio de Janeiro university who reported having been prevented from carrying out any of their usual activities (work, study, or leisure) for health-related reasons in the two weeks prior to data collection. Health problems reported in response to an open question were separately coded by two classifi ers. Interobserver reliability with respect to number of health problems was calculated by weighted kappa; for the remaining analyses (chapters and full codes), crude kappa coeffi cients were used. RESULTS:A total of 1,641 health problems were coded by the fi rst classifi er, and 1,629 by the second. Interobserver reliability with respect to the number of health problems coded was substantial (weighted kappa=0.94; 95% CI: 0.93;0.94). Chapter and full codes showed substantial (kappa=0.89; 95% CI: 0.88;0.90) and moderate (0.76; 95% CI: 0.76;0.78) reliability, respectively. CONCLUSIONS:The results suggest that the International Classifi cation of Primary Care is adequate for the coding of health-related reasons for interruption of daily activities.

OBJECTIVE:To determine reliability of data encoding for death due to violence; to assess the agreement between cause of death in the Mortality Information System and cause selected from the Forensic Medicine Institute database; and to assess the impact of adding information of non-specified injuries and undetermined death events subsequently obtained from Forensic Medicine Institute in the mortality statistics due to violence. METHODS:A random sample of 411 death certificates due to violence was obtained in Belo Horizonte, Southeastern Brazil, between 1998 and 2000. Based on data from death certificates and Forensic Medicine Institute database, causes of death were coded and the agreement between this information and that from Mortality Information System was assessed. Also, in all certificates including "non-specified injury" and "undetermined death events," the impact of adding information from Forensic Medicine Institute was assessed in the classification of cause of death. RESULTS:Coding agreement was significant (Kappa=0.782; 95% CI: 0.744; 0.819) and of the underlying cause was moderate to significant (Kappa=0.602; 95% CI: 0.563; 0.641). There were 12.9% and 5.7% misclassification of suicides and murders, respectively, for those causes classified as "non-specified injury" and "undetermined death events," which were overall reduced to 47.3% and 59.8% respectively. CONCLUSIONS:There is a need for further improving the process of underlying cause coding and selection. Also medical examiners need to provide more complete death certificates and medical and police information provided with bodies for Forensic Medicine Institute autopsy should be more complete, especially in those cases of road traffic injuries and falls.

The study presents a literature research of violence against children, intended to better understand this phenomenon and present the results to the public knowledge; the same objectives were pursued with the policies developed to deal with the violence against children, at the same time discussing the importance of the prevention and the interventions of the health professionals. The literature research was conducted on the data banks MEDLINE, LILACS and SciELO, selecting the studies with the keywords: Violence, Aggression, Child Abuse, Shaken Child Syndrome, and Pediatrics; the search was not restricted to the Spanish, English and French languages. The literature showed the importance of preventive measures and the need to discuss and reflect, among different sectors, in order to obtain policies and strategies leading to prevention, diagnostics and therapeutics. It was demonstrated the relevance of including these aspects in the training of the health professionals, which will allow them to contribute to diagnosis, treatment and prevention of child abuse, breaking the chains of predetermined fate and destiny. Keywords: Violence; Aggression; Child abuse; Battered child syndrome RESUMOO estudo apresenta uma revisão bibliográfica, cujo objetivo foi conhecer e divulgar a evolução histórica da violência contra a criança, bem como as políticas desenvolvidas na atenção à violência contra menores, além de discutir a importância da prevenção e da atuação dos profissionais de saúde. A pesquisa bibliográfica foi realizada na base de dados MEDLINE, LILACS e SciELO, selecionando-se os estudos com os descritores: Violência, Agressão, Maus-Tratos, Síndrome da Criança Maltratada, Pediatria, não se fazendo restrição aos idiomas espanhol, inglês e francês. A literatura mostra a importância de ações preventivas e a necessidade de discussões e reflexões entre os diferentes setores que possam culminar em políticas e estratégias preventivas, diagnósticas e terapêuticas, além da relevância de incluir o tema na formação dos profissionais de saúde para que possam contribuir para o diagnóstico, tratamento e profilaxia do abuso infantil, rompendo as cadeias de determinação e fatalidade.
