Introduction: Intestinal intussusception corresponds to invagination of one segment inside another. Although rare in adults, it corresponds to 1% of cases involving intestinal obstruction, being more common in the small intestine. The objective of this study is to report a case of intestinal intussusception secondary to lipoma. Case Report: female patient 51 years was admitted to the emergency room with a colicky abdominal pain for 1 month, no irradiation, and with worsening for 2 days, associated with nausea, vomiting and hematochezia. On physical examination, distended and hypertympanic abdomen with pain to palpation and rectal touch with melena was observed. Acute abdominal X-ray demonstrated evidence of colonic loop dilation and hydro-air level. Computed tomography examination of abdomen and pelvis demonstrated a target lesion suggesting intussusception, confirmed in the intraoperative by invagination from the terminal ileum to the proximal descending colon with subtotal colectomy and terminal ileostomy. Discussion: Intussusception in adults usually occurs secondary to a point of advance, either by benign tumors, malignant or motility disorders. In the large intestine, lipoma is considered a rare cause of invagination, and malignant tumors are more common, and its symptomatology and risks of complications are mainly related to the size of the lesion. Conclusion: The colic submucosal intraluminal lipoma is a rare cause of intestinal intussusception and is usually an intraoperative diagnosis.

Introduction: The invagination of one intestinal loop within another rarely occurs in adults and it is usually caused by a polyp or another intraluminal lesion. Intussusceptions occur mostly in children; an organic lesion is not necessary, and colic pain syndrome, rectal bleeding, and a palpable mass (the intussuscepted segment) are characteristics. Justification: To compare data from a tertiary reference hospital with data described in the literature, considering the evolution of such patients is unknown in our field. Objective: To report the case of a patient with distal jejunal intussusception from melanoma metastasis. Method: Information was obtained through medical record reviews, patient interviews, photographic records of the diagnostic methods and the surgery to which patients were subjected, and literature review. Final considerations: The case reported and the publications surveyed bring to light the discussion on the therapeutics of the complex condition that is melanoma metastasis, showing that some patients may present satisfactory and lasting results regarding symptom relief and improvement of quality of life.

Apresentação do caso: Paciente do sexo masculino, 46 anos de idade, com queixa de dor abdominal em hipocôndrio direito com irradiação para flanco esquerdo de grande intensidade, acompanhada de emagrecimento e plenitude. Exames laboratoriais realizados não mostraram alterações. A ultra-sonografia abdominal mostrou presença de volumosa coleção com debris em flanco direito e alças instestinais com peristalse, que sugeria o quadro de invaginação intestinal (imagem similar a "alça dentro de alça"). Na colonoscopia, notou-se tumoração multinodular gigante projetada sobre mucosa com aspecto de intussuscepção do cólon proximal. O paciente foi submetido a uma laparotomia exploradora mediana na qual se observou invaginação de aproximadamente 10 cm de íleo terminal para dentro do colón direito. A conduta cirúrgica foi redução da invaginação e enterectomia com enterocoloanastomose mecânica término-terminal e apendicectomia profilática. O estudo anatomopatológico da peça cirúrgica mostrou segmento de intestino delgado de formato habitual, apresentando edema, vasodilatação e infiltrado inflamatório predominantemente redondonuclear. O apêndice possuia formato habitual, revestido por serosa opaca com depósito fibrino de coloração pardo-amaronzada, consistência elástico-borrachosa. Portanto, indicando periapendicite reacional na ausência de sinais de malignidade. Discussão: A intussuscepção intestinal é uma entidade rara em adultos, e seu mecanismo exato é desconhecido, porém acredita-se que a presença de lesão na parede intestinal altera o peristaltismo normal iniciando o processo de invaginação. A presença de lesão orgânica tem sido descrita em 70 a 90% dos casos de intussuscepção em adultos, sendo maioria malignas. O sintoma mais comumente observado é a dor abdominal. A sintomatologia é geralmente inespecífica e crônica, sugerindo obstrução intestinal na maioria das vezes. Os exames de imagem não foi identificaram a causa da intussuscepção, e o diagnóstico de certeza somente foi dado no intra-operatório, com o achado de invaginação ileocecal. Comentários finais: O tratamento cirúrgico envolve a ressecção dos segmentos intestinais acometidos, devido ao maior risco da presença de neoplasias malignas como causa da intussuscepção do segmento. O estudo anatomopatológico não demonstrou sinais de malignidade, apoiando então, o diagnóstico de intussuscepção idiopática, entidade nosológica rara em adultos.
