Lymph node metastasis in early gastric cancer

Abstract: the incidence of lymph node metastases in patients was very high and suggests that one should keep the radicality of resection in early gastric cancer, particularly in relation to D2 lymphadenectomy, recommended for advanced gastric cancer. Conservative resections, with lymphadenectomies smaller than D2, should be performed only in selected cases, well-studied as for the risk factors of lymph node metastasis. Despite the small number of cases did not permit to relate the rate of lymph node metastasis to the ri… Show more

“…Univariate analysis showed that tumor size, the presence of middle and lower stomach cancer, poorly differentiated adenocarcinoma, signetring cell carcinoma, diffuse type cancer, and lymphatic invasion correlated with lymph node metastasis. Multivariate analysis showed that lymphatic invasion and tumor size were significant predictors of lymph node metastasis [13]. Similar findings were also observed in the present study.…”

“…The results demonstrated that RP11-119F7.4 had potential diagnostic value in gastric cancer and further indicated that increased expression levels tended to be associated with Borrmann type IV and diffuse-type gastric cancer. A number of previous studies have indicated that Borrmann type IV and diffuse Lauren-type gastric cancers were associated with a worse prognosis compared with other types (32,33). According to the results of the present study, RP11-119F7.4 expression levels may be associated with tumorigenesis and the level of malignancy in gastric cancer.…”

“…However, many studies from Japan and other countries have not demonstrated significant differences in the rate of LNM between early PGC (i.e., carcinoma in the upper one‐third stomach in Japan) and DGC. This may be due to the small sample sizes of PGC . The recent discovery of different genomic profiles of PGC, compared to those of DGC, may explain why early PGC, unlike early DGC, exhibits a very low risk of LNM and therefore may be more suitable than early DGC for endoscopic resection.…”
