Ultrasound-based methods have shown promise in their ability to characterize non-uniform deformations in large energy-storing tendons such as the Achilles and patellar tendons, yet applications to other areas of the body have been largely unexplored. The noninvasive quantification of collateral ligament strain could provide an important clinical metric of knee frontal plane stability, which is relevant in ligament injury and for measuring outcomes following total knee arthroplasty. In this pilot cadaveric experiment, we investigated the possibility of measuring collateral ligament strain with our previously validated speckle-tracking approach, but encountered a number of challenges during both data acquisition and processing. Given the clinical interest in this type of tool, and the fact that this is a developing area of research, the goal of this article is to transparently describe this pilot study, both in terms of methods and results, while also identifying specific challenges to this work and areas for future study. Some challenges faced relate generally to speckle-tracking of soft tissues (e.g. the limitations of using a 2D imaging modality to characterize 3D motion), while others are specific to this application (e.g. the small size and complex anatomy of the collateral ligaments). This work illustrates a clear need for additional studies, particularly relating to the collection of ground-truth data and more thorough validation work. These steps will be critical prior to the translation of ultrasound-based measures of collateral ligament strains into the clinic.

Aim. To assess the morphometric characteristics of Achilles tendons in patients with familial hypercholesterolemia (FH) and to identify factors associated with an increase in their size.Материал и методы. Totally, 100 patients included, with severe primary hyperlipidemia, defined as an increase in total cholesterol ≥7,5 mM/L and/or lowdensity lipoprotein cholesterol ≥4,9 mM/L. All patients underwent duplex scanning of carotid arteries. Ultrasound examination of the Achilles tendons was carried out on an expert class ultrasound equipment Samsung Medison EKO 7 (Japan) with a linear sensor, frequency of 7-16 MHz. Thickness of the Achilles tendon (anteriorposterior dimension (APD)) was measured during scanning in the longitudinal section, width (transverse dimension (TD)) — scanning in the cross section. The measurements were made 2 cm proximal to the calcaneus.Results. Among the participants, 32 (32,0%) had definite/probable diagnosis of FH. In patients with definite/probable FH the mean values of APD were significantly higher in comparison with patients scored 5 or less points (DLCN) — 5,50 (4,70-6,10) mm vs. 5,00 (4,50-5,40), respectively (p=0,04). TD of Achilles tendons among this category of patients was also statistically significantly higher in comparison with the rest of patients — 14,0 (12,9-15,4) mm and 13,2 (12,2-14,2) mm, respectively (p=0,04). In correlation analysis, the relationship between the growth of patients and the APD of the Achilles tendons (r=0,34, p=0,001), the TD of the Achilles tendon (r=0,28, p=0,009), CIMT and TD of the Achilles tendons (r=0,21, p=0,05), amount of carotid plaques and TD of the Achilles tendon (r=0,26, p=0,01), total percentage of stenosis of the carotid arteries and the TD of the Achilles tendons (r=0,27, p=0,01), maximum percentage of stenosis of the carotid arteries and the TD of the Achilles tendons (r=0,28, p=0,007). According to regression analysis, factors associated with an increase in thickness of the Achilles tendons for more than 75 percentiles were male sex, diabetes mellitus, height, myocardial infarction in relatives, total percentage of stenosis of the carotid arteries and high-density lipoproteide cholesterol.Conclusion. In patients with definite/probable FH, mean values of width and thickness of the Achilles tendons were significantly higher in comparison with the rest of the patients. According to regression analysis, the factors associated with the increase in thickness of the Achilles tendons fro more than 75 percentiles were male sex, diabetes mellitus, height, myocardial infarction in relatives, total percentage of stenosis of the carotid arteries and HDL high-density lipoproteide cholesterol.

RESUMEN:El tendón calcáneo (TC) en el hombre es el más voluminoso y resistente del cuerpo, constituyéndose por su inserción en un elemento vital en la mecánica articular del pie como también en las numerosas patologías que afectan las regiones talocrural y calcánea. Para el estudio se utilizaron 120 miembros inferiores (60 derechos y 60 izquierdos), de cadáveres formolizados de individuos adultos, de ambos sexos. Se disecó la cara posterior de la pierna desde la línea articular de la rodilla hasta la parte inferior del calcáneo, se retiró el tejido adiposo precalcáneo dejando libre el TC. Se observó que el TC estaba formado, exclusivamente, por la fusión de las aponeurosis de los músculos gastrocnemio y sóleo en 62 casos (51,67%). El tendón del músculo plantar contribuye a la formación parcial del TC en 38 casos (31,67%) e integralmente en 20 casos (16,66%). El ancho del TC a nivel de la terminación del vientre del músculo sóleo fue de 12,88 mm ± 2,0 en el hombre y de 11,55 mm ± 1,6 en la mujer y el grosor del TC al mismo nivel de 4,29 mm ± 0,64 en el hombre y de 4,36 mm ± 0,64 en la mujer. El ancho del TC a nivel del margen posterosuperior del calcáneo fue de 17,98 mm ± 1,7 en el hombre y de 17,06 mm ± 1,53 en la mujer. El grosor del TC al mismo nivel fue de 3,79 mm ± 0,61 en el hombre y de 3,93 mm ± 0,67 en la mujer. La distancia entre el margen posterosuperior del calcáneo y el inicio de la parte insertada del TC fue de 10,99 mm ± 2,11 en el hombre y 10,84 ± 2,71 en la mujer. El largo de la parte insertada del TC fue de 17,78 mm ± 2,4 en el hombre y de 17,66 mm ± 4,75 en la mujer. El ancho del TC a nivel de su inserción en el hueso calcáneo fue de 28,77 mm ± 2,53 en el hombre y de 27,21 mm ± 2,93 en la mujer. La tendinopatía en la inserción del TC suele ser frecuente requiriendo en algunos casos debridamiento quirúrgico del tendón. Por lo anterior, conocer aspectos morfométricos del TC y de su inserción adquiere importancia en los procedimientos quirúrgicos de la región talocrural posterior.PALABRAS CLAVE: Anatomía; Tendón calcáneo. INTRODUCCIÓNEl tendón calcáneo es una de las pocas estructuras anatómicas que la Terminología Anatómica internacional acepta con más de una denominación, contrario a las propias normas establecidas incluso en la propia Nomina Anatomica de 1895. Así, este tendón sigue siendo denominado con el famoso nombre mitológico de Aquiles. La leyenda nos cuenta que la madre de Aquiles lo aseguró por los tobillos para sumergirlo en el río Estige y así tornarlo invulnerable, sin embargo, murió en el cerco de la ciudad de Troya cuando fue herido mortalmente por una flecha en el TC.Las lesiones agudas del TC, provocan rotura del mismo y se dan preferentemente en atletas de alto nivel. Por otra parte, las lesiones crónicas que afectan a este tendón, como las tendinitis, se produce por la carga continua del tendón, especialmente en aquellos que practican atletismo, corredores fondistas e incluso velocistas.El TC está constituido por la fusión de las aponeurosis de los músculos gastrocnemios y sóleo...
