RESUMO Objetivo Comparar a evolução das vocalizações em bebês prematuros e a termo, com e sem risco ao desenvolvimento, analisando as possíveis relações entre variáveis sociodemográficas, obstétricas e psicossociais com as vocalizações. Método A amostra foi composta por 30 bebês com idade entre os 3 meses e 1 dia aos 4 meses e 29 dias (fase 1) e 6 meses e 1 dia aos 7 meses e 29 dias (fase 2), de ambos os gêneros, com idade gestacional inferior a 37 semanas (grupo de prematuros) e superior a 37 semanas (grupo a termo). Para a coleta de dados, utilizaram-se os protocolos Indicadores de Risco ao Desenvolvimento Infantil, o teste Denver II e entrevista sobre a experiência da maternidade com dados sociodemográficos, obstétricos e psicossociais, além de filmagem da díade mãe-bebê nas duas fases da pesquisa. Os dados das filmagens foram analisados no software EUDICO Linguistic Anotador (ELAN) e os resultados analisados estatisticamente no software STATISTICA 9.0. Resultados Quanto maior o número total de vocalizações do bebê e quanto mais vocalizações das mães com manhês, maior o número de Indicadores de Risco ao Desenvolvimento Infantil presentes. Também se percebeu aumento significativo de vocalizações sem manhês na fase 2 pesquisada. As variáveis sociodemográficas, idade gestacional, peso ao nascer, escolaridade materna e o Critério Brasil não incidiram diretamente no nível de vocalizações dos bebês. Conclusão A análise das vocalizações dos bebês associou-se ao risco ao desenvolvimento, assim como os Indicadores de Risco ao Desenvolvimento Infantil, na fase 1 pesquisada, o teste Denver-Linguagem é mais efetivo na fase 2. Não houve influência das variáveis sociodemográficas na fase estudada.

Evolução comunicativa em gemelares com atraso no desenvolvimento da linguagem

Este estudo tem como tema o comportamento comunicativo de gêmeos com alteração de linguagem pré e pós intervenção fonoaudiológica. Foram analisados dados de dois pares de gêmeos do sexo masculino (S1/S2 monozigóticos; S3/S4 dizigóticos), com diagnóstico de Atraso de Linguagem. Os dados foram coletados na avaliação pré e pós intervenção fonoaudiológica, por meio de: entrevista semi-estruturada com os pais; observação do comportamento comunicativo; e teste de linguagem e escala de desenvolvimento. A intervenção fonoaudiológica teve duração de 34 meses para S1/S2 (de dois anos e cinco meses a cinco anos e três meses) e 17 meses para S3/S4 (de três anos e sete meses a cinco anos e quatro meses). Na avaliação pré intervenção fonoaudiológica de S1/S2 foram observadas intercorrências pré, peri e /pós natais mais graves, com maior comprometimento neuropsicomotor e comunicativo. S3/S4 apresentaram intercorrências pré, peri e pós natais menos graves e o atraso no desenvolvimento ocorreu apenas para a linguagem. A idade das crianças na avaliação pós intervenção foi semelhante, porém, o par S1/S2 apresentou desempenho comunicativo e global com maior prejuízo, mesmo tendo recebido maior tempo de intervenção fonoaudiológica. S3 apresentou desempenho aproximado ao desenvolvimento típico e S4 dificuldades apenas quanto à fonologia e pragmática. As experiências de gemelares, diferenciadas em meio a circunstâncias interpessoais, acarretam desenvolvimento de linguagem atípico, devido à menor necessidade objetiva de interação linguística. As intercorrências pós natais em S1/S2 agravaram o atraso no desenvolvimento infantil, englobando o desenvolvimento comunicativo.

Objective: To determine chronological and corrected ages at acquisition of motor abilities up to independent walking in very low birth weight preterms and to determine up to what point it is necessary to use corrected age. Methods:This was a longitudinal study of preterms with birth weight < 1,500 g and gestational age ≤ 34 weeks, free from neurosensory sequelae, selected at the high-risk infants follow-up clinic at the Hospital das Clínicas, Faculdade de Medicina de Botucatu, Universidade Estadual Paulista (UNESP) in Botucatu, Brazil, between 1998 to 2003, and assessed every 2 months until acquisition of independent walking.Results: Nine percent of the 155 preterms recruited were excluded from the study, leaving a total of 143 patients. The mean gestational age was 30±2 weeks, birth weight was 1,130±222 g, 59% were female and 44% were small for gestational age. Preterms achieved head control in their second month, could sit independent at 7 months and walked at 12.8 months' corrected age, corresponding to the 4th, 9th and 15th months of chronological age. There were significant differences between chronological age and corrected age for all motor abilities. Preterms who were small for their gestational age acquired motor abilities later, but still within expected limits. Conclusions:Very low birth weight preterms, free from neurosensory disorders, acquired their motor abilities within the ranges expected for their corrected ages.
