The incidence of Candida bloodstream infection (BSI) has increased during the past decades. Species distribution is changing worldwide, and non-albicans Candida spp. are becoming more prevalent. Acquired resistance to antifungal agents has been documented in several reports. The aim of our study was to assess the epidemiology and antifungal susceptibility of Candida isolates from BSI at our institute. The incidence of Candida BSI increased during the fi rst four years of our investigation, from 1.7 to 3.5 episodes / 10 000 admissions, then dropped to 2.66 episodes / 10 000 admissions in the last year. The most frequently isolated species was C. albicans (63%), followed by C. glabrata (13%), C. parapsilosis (10.2%), C. tropicalis (9.3%), and C. krusei (3.7%). One isolate each of C. kefyr, C. fabianii and C. inconspicua were detected. The percentage of C. albicans remained stable throughout the study period. The most frequent risk factors of Candida BSI in our patient population were intensive care treatment (60.4%), abdominal surgery (52.5%), and solid malignancy (30.7%). All isolates were wild-type organisms, no acquired antifungal resistance was detected.

dIn recent years, the emergence of fungal resistance has become frequent, partly due to the widespread clinical use of fluconazole, which is minimally toxic and effective in the prevention and treatment of Candida albicans infections. The limited selection of antifungal drugs for clinical fungal infection therapy has prompted us to search for new antifungal drug targets. Calcium, which acts as the second messenger in both mammals and fungi, plays a direct role in controlling the expression patterns of its signaling systems and has important roles in cell survival. In addition, calcium and some of the components, mainly calcineurin, in the fungal calcium signaling pathway mediate fungal resistance to antifungal drugs. Therefore, an overview of the components of the fungal calcium-calcineurin signaling network and their potential roles as antifungal targets is urgently needed. The calciumcalcineurin signaling pathway consists of various channels, transporters, pumps, and other proteins or enzymes. Many transcriptional profiles have indicated that mutant strains that lack some of these components are sensitized to fluconazole or other antifungal drugs. In addition, many researchers have identified efficient compounds that exhibit antifungal activity by themselves or in combination with antifungal drugs by targeting some of the components in the fungal calcium-calcineurin signaling pathway. This targeting disrupts Ca 2؉ homeostasis, which suggests that this pathway contains potential targets for the development of new antifungal drugs.

RESUMOCandida sp. é o agente fúngico mais frequentemente isolado e associado a altas taxas de morbimortalidade em pacientes hospitalizados. Apesar de bem estabelecida, a epidemiologia dessas infecções tem apresentado grande variação em relação à região e ao período estudado. O objetivo desse trabalho foi avaliar a incidência e a distribuição de espécies de Candida em quatro cidades da região metropolitana do estado do Rio de Janeiro, no período de 2013 a 2014. Um total de 100 cepas foi recuperado de diferentes estabelecimentos de saúde das áreas investigadas. Para a identificação dos isolados, ABSTRACTCandida sp. is the most frequently isolated fungal agent and associated with high morbidity and mortality rates in hospitalized patients. Although well-established, the epidemiology of infections has shown wide variation in relation to the region and the period studied. The objective of this study was to evaluate the incidence and distribution of Candida species in four cities in the metropolitan region of Rio de Janeiro state in the period from 2013 to 2014. A total of one hundred strains were recovered from different health facilities of the investigated areas. For the identification of isolates, phenotypic tests (micromorphology on CHROMagar, germ tube and chlamydospore production) and mass spectrometry (MALDI-TOF/MS) were used. Of all the isolates, 68,0% belonged to the Candida non-albicans group and 32,0% belonged to the Candida albicans specie. Regarding clinical specimens, 44,0% of the studied strains were isolated from urine samples, followed by blood culture (13,0%). In this study, the hospital sector with the highest fungal recovery rate was the Intensive Care Unit, with 40% of the isolates. Among the four evaluated regions, the city of Niteroi was the one with the highest percentage of isolation (52,0%). 17,18,19,20 .
