The present survey from the State of Alagoas (Brazil) emphasized, once again, the importance of peridomestic or neighbourhood foci in the transmission of schistosomiasis mansoni. Although the study area consisted of a compact and densely populated urban community (Bairro Frio, União dos Palmares county), prevalence as well as the distribution of high egg counts was quite patchy, exhibiting a spatial pattern correlated with the distribution of ditches and other small-sized collections of surface water. Despite the availability of satisfactory water supply in most households, as well as the absence of any major body of water capable of furthering transmission, egg counts in Bairro Frio exceeded any other in our previous experience. Morbidity, however, appeared to be insignificant, a trend which has been observed over the last decade in several other highly endemic areas. It appears clear to the authors that the control of transmission requires investment in environmental intervention and that this proposal, far from being utopic, can be furthered by quite modest means and through the participation of community or neighbourhood groups.

“…It will be important to follow the pathology of S. mnnsoni infection in this area by ultrasound studies to determine if and when DPH attributable to S. mansoni infection becomes a problem. Treatment with praziquantel may be used to prevent hepatic complications as suggested by Andrade and Bina (1985). We found an important amount of periportal thickening ( 3 3 %) and parenchymal lesions ( I I %) in these subjects before treatment, only 3 years after the first case had been detected (Rouquet et al 1993).…”

SummaryFollowing the introduction of large-scale irrigation, an exceptional epidemic of intestinal schistosomiasis occurred in northern Senegal when a non-immune population was exposed to massive infection. Subjects infected with Schistosoma mansoni were followed up parasitologically and clinically from the onset of the epidemic. After the initial evaluation, patients received a health education session and were treated with praziquantel in a dose of 3 0 mdkg.One year after this treatment, S. mansoni eggs were found in the stools of zz7/301 subjects ( 7 5 % ) . Twenty-three per cent of subjects excreted >400 eggs per gram (e.p.g.) and 11% excreted >IOOO e.p.g. of faeces. Overall, the geometric mean was 191 e.p.g. of faeces in infected individuals. The prevalence of diarrhoea was reduced from 5 5 to 29%, the prevalence of bloody diarrhoea from 44 to I T % and the prevalence of abdominal discomfort from 66 to 41%. N o hepatomegaly was found in these patients either before or one year after treatment. Splenomegaly was reduced from 30% (measured by ultrasound) to 3 Yo (on clinical examination). Morbidity associated with S. mansoni infection was considerably reduced one year after treatment with praziquantel ( 3 0 mdkg).
