The changing pattern of pathology due to Schistosoma mansoni infection

Abstract: A survey of the autopsy data on hepatosplenic schistosomiasis during periods, before and after the advent of new chemotherapeutic drugs, revealed that: a) the pathological presentation was the same for the two periods; b) the number of cases in the last five years is progressively decreasing; c) hepatosplenic disease due to schistosomiasis is becoming rare in young people. These data represent a change in the pattern of pathology in schistosomiasis, probably related to new chemotherapy.

“…At the same time, from 115 non-treated controls, 21 developped hepatosplenic disease and 15, previously presenting the hepatosplenic form of the disease, either remained as such or deteriorated further. These interesting results led Andrade and Bina (1985) to analyze necropsy protocols from the University Hospital at Salvador, Bahia. They observed that during 20 years (1956)(1957)(1958)(1959)(1960)(1961)(1962)(1963)(1964)(1965)(1966)(1967)(1968)(1969)(1970)(1971)(1972)(1973)(1974)(1975)) the percentage of necropsies performed on cases of hepatosplenic schistosomiasis maintained a constant level, between 6-7% annually.…”

“…Since 1985, when the present series of studies in Alagoas began, we have been puzzled by the present benignity of the clinical picture displayed by schistosomiasis, a finding which was confirmed by other authors (Andrade & Bina, 1985;Menezes-Neto, 1987). This pattern is in vivid contrast to the classical clinical description of hepatosplenic schistosomiasis, formerly so prevalent in many parts of Brazil (Meira, 1951).…”

“…It will be important to follow the pathology of S. mnnsoni infection in this area by ultrasound studies to determine if and when DPH attributable to S. mansoni infection becomes a problem. Treatment with praziquantel may be used to prevent hepatic complications as suggested by Andrade and Bina (1985). We found an important amount of periportal thickening ( 3 3 %) and parenchymal lesions ( I I %) in these subjects before treatment, only 3 years after the first case had been detected (Rouquet et al 1993).…”
