Valvoplastia mitral percutânea por balão: evolução a longo prazo e análise dos fatores de risco para óbito e eventos maiores

Abstract: Percutaneous mitral balloon valvotomy is an effective procedure, and over 2/3 of the patients were event-free at the end of follow-up. Survival in the group was high, even higher in the group with lower echocardiographic scores.

“…The occurrence of severe mitral regurgitation during the procedure predicted events during long term evolution in other studies 11,12,13,19,22,32,35 and mitral regurgitation can be predicted by a specific echocardiographic score 36 . Mitral regurgitation before the valvuloplasty is a predictor of reduced event free survival 11 .…”

“…But even though the results were not as positive, the group with echocardiographic scores > 8 also presented satisfactory results and evolution 13,18 , particularly those with a score < 11 13 . In the single and Inoue balloon groups, the echocardiographic scores presented similar averages even though the percentage of patients with a score > 8 was higher in the Inoue balloon group.…”

“…In previous studies we discovered during the long term evolution that surgical valve repair was not an independent factor to predict death, however it was an indication for major events and a higher number of reinterventions [30][31][32] . Others have shown the infl uence of the Wilkins 27 echocardiograph score in the mitral valvuloplasty results 29,49,54 , for both immediate results 29,49,54 and long term evolution 30,31,[54][55][56] , and the group of patients that had previously undergone valve repair procedures presented a higher echocardiograph score.…”

“…Generally speaking, the studies documented in medical literature report similar results with the techniques mentioned [8][9][10][11][12][13][24][25][26] . In our case, the results attained in patients who had previously undergone commissurotomy surgery depended on the state of the mitral valve (Wilkins score) 27 and not on the previous surgical treatment; however, despite the positive immediate results 28,29 , and satisfactory evolution, the fi rsttime group presented a better evolution [30][31][32] .…”
