Background Closure of perimembranous ventricular septal defects (pmVSD), either surgical or percutaneous, might improve or cause new-onset mitral regurgitation (MR) and aortic regurgitation (AR). We aimed to evaluate the changes in MR and AR after pmVSD closure by these two methods. Material and method We performed a comparative retrospective data review of all pediatric patients with pmVSDs treated at our institution with surgical or antegrade percutaneous methods from 2014 to 2019 and 146 consecutive patients under 18 years were enrolled. We closely looked at the mitral and aortic valve function after repair. Included patients had no or lower than moderate aortic valve prolapse and baseline normal mitral or aortic valve function or less than moderate MR or AR. Results Out of 146 patients, 83 (57%) pmVSDs were closed percutaneously, and 63 (43%) pmVSDs were closed surgically. 80 and 62 patients were included for MR evaluation, and 81 and 62 patients for AR evaluation in percutaneous and surgical groups. The mean follow-up time was 40.48 ± 21.59 months in the surgery group and 20.44 ± 18.66 months in the transcatheter group. Mild to moderate degrees of MR and AR did not change or decreased in most patients. In detail, MR of 70% and AR of 50% of the valves were resolved or decreased in both groups. 13% of patients with no MR developed trivial to mild MR, and 10% of patients with no AR showed mild to moderate AR after pmVSD closure in both methods. There was no significant difference between the two methods regarding emerging new regurgitation or change in the severity of the previous regurgitation. Conclusion pmVSD closure usually improves mild to moderate MR and AR to a nearly similar extent in both percutaneous and surgical methods among children and adolescents. It might lead to the onset of new MR or AR in cases with no regurgitation.

Background: Doppler echocardiography is a reliable and non-invasive method of detecting valvar regurgitation. The aim of this study was to determine the prevalence of valvar regurgitation in children with structurally normal hearts and explore its relationship with age, gender and anthropometry. Methods: This was a descriptive cross-sectional study conducted in four tertiary hospitals in Nigeria. Two hundred and thirty-three children (124 males and 109 females), from birth to 18 years were recruited prospectively. Using transthoracic echocardiography, the presence of valvar regurgitation was assessed across the four cardiac valves using Color Doppler interrogation after structural abnormalities were ruled out. Data was analyzed using the Statistical Package for Social Sciences (SPSS) software, version 22. Results: Valvar regurgitation was found in 158/233 children giving a prevalence of 67.8%. They consisted of 87 males and 71 females with a male: female ratio of 1.2:1. Of these, 82 (35.2%) had a single valvar regurgitation, 72 (30.9%) had regurgitation at two valves while 4 (1.7%) had regurgitation at three valves. Pulmonary regurgitation was the most common in 52.8% of cases while aortic regurgitation was seen in only two children (0.9%). There was a non-signifi cant negative correlation of age and body surface area with presence of tricuspid regurgitation (ρ-0.79, p=0.22 and ρ-0.12, p=0.08) and mitral regurgitation (ρ-0.04, p=0.56 and ρ-0.02, p=0.83) but positive correlation with pulmonary regurgitation (ρ 0.11, p= 0.11 and ρ 0.12, p= 0.08). The presence of valvar regurgitation was not associated with gender (p=.>0.05). Conclusion: The prevalence of valvar regurgitation in apparently healthy Nigerian children is 67.8%. The presence of valvar regurgitation especially of the right sided valves could be a physiologic fi nding in apparently healthy children with structurally normal hearts. Regurgitation of the left sided valves is rare.

Problem statement: Valvular regurgitation is recognized as the central cause of morbidity and mortality. Even though the clinician can detect the presence of regurgitation by mere physical examination; diagnostic methods become inevitable while estimating the severity of valvular regurgitation and in the transformation of cardiac chambers as in reaction to the volume overload condition. Lately, a promising new technology, the Echocardiography with Doppler is found to facilitate the non-invasive recognition and assessment of the severity and etiology of valvular regurgitation. Accurate measurements of regurgitant volume in patients is of utmost importance since it aids in the estimation of the progression of the disease which in turn is vital for determining the optimal time for surgical repair or replacement. Approach: Color space conversion and anisotropic diffusion segmentation techniques are utilized in this study for the pre-processing stage of the quantification of mitral regurgitation. Flow field measurements are carried out with the aid of proximal flow convergence method. Results: A calculated value of flow rate, regurgitant orifice area, regurgitant fraction and the regurgitant volume for a regurgitant orifice in the cardiovascular system are obtained from the potential Color Doppler visualization of the flow convergence region. Conclusion:The research proposed provides a significant assessment of the echocardiographic and Doppler techniques employed in the evaluation of mitral valvular regurgitation in the patients. Additionally it also proffers the estimation of mildness, severity and eccentricity of mitral valvular regurgitation on basis of the scientific literature and a consensus of a panel of experts.
