Aim: Obsessive-compulsive disorder (OCD) impacts family functioning as family members modify their personal and family routines, participate in rituals, and provide reassurance. These behaviors have been identified as family accommodation (FA), a phenomenon that, if ignored, may facilitate OCD symptoms and lead to poorer prognosis. Because FA has been recognized as a predictor of treatment outcome, we examined the prevalence of FA and identified patient and family sociodemographic and clinical variables associated with FA in an outpatient sample. Methods:The study comprised 228 subjects, namely, 114 patients with OCD and 114 family members, assessed before the patients entered a 12-session cognitive behavioral group therapy program. A multivariate linear regression model was used to control for confounding factors and to evaluate variables independently associated with FA. FA was evaluated using the Family Accommodation Scale for Obsessive-Compulsive Disorder -Interviewer Rated.Results: FA was found to be highly prevalent among family members. Two patient factors positively associated with FA were OCD severity as measured by the Clinical Global Impressions Scale and higher scores on the Obsessions dimension of the ObsessiveCompulsive Inventory -Revised. Family members' characteristics that were positively associated with FA were higher scores on the Obsessive-Compulsive Inventory -Revised hoarding subscale and being the patient's spouse. Conclusion:Our findings suggest that the early identification of patients and family members who could benefit from interventions aimed at reducing FA could improve treatment outcomes.

RESUMO: Trata-se de estudo retrospectivo e exploratório, realizado em instituição psiquiátrica com o objetivo de caracterizar os pacientes acometidos por Transtorno Obsessivo Compulsivo quanto ao sexo, idade, proveniência, tempo de internação, tipo de alta, tipo de internação e fonte financiadora da internação. A amostra foi constituída por 61 pacientes sendo predominante o sexo feminino (47-77%), a faixa etária acima de 41 anos (41-67%), provenientes da própria família (31-50,8%) e o tempo de internação de 30 dias (33-54,1%). A alta médica foi a mais frequente (51-83,6%), houve prevalências similares entre as primeiras internações (30-49,2%) e reinternações (31-50,8%); o principal financiador das internações hospitalares foi o Sistema Único de Saúde (54-88,5%). Ações que visem diagnosticar precocemente o transtorno devem ser desenvolvidas para que o tratamento seja prontamente iniciado a fim de evitar o agravamento de obsessões e compulsões.ABSTRACT: This is a retrospective, exploratory study, undertaken in a psychiatric institution with the aim of characterizing the patients affected by Obsessive-Compulsive Disorder by sex, age, provenance, duration of hospitalization, type of discharge, type of hospitalization, and source of financing of hospitalization. The sample comprised 61 patients, the majority of whom were female (47-77%), age range over 41 years old (41-67%), who came to hospital from the family home (31-50.8%) and length of hospitalization of 30 days (33-54.1%). The most frequent type of discharge was medical (51-83.6%), and there were similar prevalences for first hospitalizations (30-49.2%) and re-hospitalizations (31-50.8%); the main financer for hospitalization was the Unified Health System (54-88.5%). Actions for the early diagnosis of this disorder must be developed so that the treatment may be initiated promptly, so as to avoid the worsening of obsessions and compulsions. ResumoMaria, 35 anos, iniciou tratamento psicológico por indicação de seu médico psiquiatra e apresentava sintomas obsessivo-compulsivos [OCs] severos, com significativo prejuízo no seu funcionamento social, ocupacional e na sua saúde. Suas obsessões eram associadas à ideia de contaminação e doença, e suas compulsões eram de limpeza e higiene. Apresentava também comportamentos de esquiva associados às obsessões e reduzido repertório social. A história de vida e a avaliação psicológica de Maria permitiram compreender a evolução de seu quadro sintomatológico e a identificação dos aspectos predisponentes, precipitadores e mantenedores do transtorno obsessivo-compulsivo [TOC]. Baseado no registro realizado durante os atendimentos psicológicos, com o tratamento cognitivo-comportamental, observou-se uma redução na frequência dos comportamentos compulsivos. A auto avaliação da cliente e a observação clínica também apontaram uma melhora no funcionamento social e em sua condição física. No entanto, não foi possível realizar o follow-up para averiguar permanência dos ganhos obtidos durante o tratamento. Palavras-chave:Comportamento; Obsessivo; Terapia AbstRAct Maria, aged 35, began psychological treatment after referral by her psychiatrist, presenting severe obsessive-compulsive symptoms [OCs] which had a significantly adverse impact on her social and occupational function and on her health. Her obsessions were associated with the idea of infection and illness and her compulsions were cleanliness and hygiene. She also presented avoidance behaviors associated with these obsessions and deficit in social skills. Maria's life history and her psychological assessment enabled us to understand the development of her symptoms and to identify the predisposing, precipitating and maintaining aspects of the obsessive-compulsive disorder [OCD]. Based on the records kept during the cognitive-behavioral treatment, a reduction in the frequency of her compulsive behavior was observed. The client's self-assessment and the clinical observations also pointed towards improvement in social function and physical condition. Future efforts should include a follow-up to check for the maintenance of these gains. CASE CONTEXT AND METHOD The RaTionale foR SelecTing ThiS PaRTiculaR clienT STudyThere were four main reasons that determined the choice of Maria's case for this study: the OCD diagnosis, the severity of the OC symptoms, the lack of prior psychological treatment, and the cognitive-behavioral treatment carried out. This case illustrates the incapacitation caused by OC symptoms and the influence of life history on the development and permanence of the disorder. An analysis of the records kept during the psychological therapy session also allowed us to ascertain the progress made with the cognitive-behavioral treatment.
