Abstract:Background: Infertility, reported commonly in gynecological practice, has many dimensions. There are many factors, visible / invisible and health seeking practices are also varied. Setting of Study: Department of Obstetrics and Gynaecology, Mahatma Gandhi Institute of Medical Sciences, Sevagram, Wardha, Maharashtra India. Objective: To find out knowledge of women about conception events, gross causes of infertility and the advice-seeking scenario of women with infertility. Subjects: Prospective study was done over two years. Material: One thousand (532 rural & 468 urban) women were interviewed by social worker. Intervention: Analysis of information about duration of married life, problems in husband, wife and advice-seeking practices. Results: Women had little knowledge about conception events, possible treatment options for infertility, but were enthusiastic to know. Personnel from whom treatment was sought varied from quacks to specialist care-giver as entry place. Summary and Conclusion: Gross causes of infertility were tubal factors, anovulation, endocrine disorders and uterine causes. Infertile women from poor resource settings are not aware of possible causes of infertility. Some seek care from quacks, others visit specialist doctors as the first contact. Infertility therapy must encompass information, education and counseling also. Many women may need only awareness, counseling and no other medical/surgical interventions.

“…In men with prior vasectomy, microsurgical reconstruction of the reproductive tract is more cost-effective than sperm retrieval with in vitro fertilization and intracytoplasmic sperm injection if the obstructive interval is less than 15 years and no female fertility risk factors are present. If epidid- ymal obstruction is detected or advanced female age is present, the decision to use either microsurgical reconstruction or sperm retrieval with in vitro fertilization and intracytoplasmic sperm injection should be individualized [16] . Patients who had inguinal vasal obstruction were obstructed for a longer period of time and the procedure is more demanding than vasectomy reversal.…”

Introduction: It was the aim of this study to assess whether the changes in the diagnostic techniques and treatment modalities have altered the epidemiology of male factor infertility in the last decade. Material and Methods: From September 1999 to July 2003, 822 patients were evaluated for infertility in a University Hospital. We divided our infertility patients according to the clinical diagnosis. Results: Most of the patients presented with varicocele (n = 282, 34.3%), idiopathic infertility (n = 260, 31.6%), or had had seminal tract obstruction (n = 85, 10.34%). Least common but equally important causes found were mumps (n = 43, 5.23%), pyospermia (n = 37, 4.5%), systemic diseases (n = 36, 4.37%), testicular failure (n = 34, 4.13%), cryptorchidism (n = 14, 1.7%), ejaculatory dysfunction (n = 11, 1.3%), genetics (n = 9, 1.1%), endocrinopathies (n = 4, 0.5%), testicular cancer (n = 4, 0.5%), and testicular torsion (n = 3, 0.36%). Conclusions: Even with the changes in reproductive healthcare in the last years, varicocele and seminal tract obstruction remain the leading causes of male infertility. However, clinicians should not forget other treatable causes of male infertility such as pyospermia, systemic diseases, or testicular cancer.

Bu çalışmada erişkin sıçanlarda vazektomi sonrası seminifer tübül epitelindeki yapısal değişiklikleri mikroskobik olarak incelemek ve nitel ve nicel olarak değerlendirmek amaçlanmıştır. Gereç ve Yöntemler: Toplam 48 adet 8 haftalık erkek Wistar albino sıçan kullanıldı. Çalışma deney grupları ve kontrol grupları içerdi (her grup için, n=8). Deney gruplarında tek taraflı vazektomi uygulanırken, kontrol gruplarında yalnızca abdominal kesi yapıldı. Deney ve kontrol gruplarındaki sıçanların sol testisleri vazektomiden 1, 12 ve 24 hafta sonra perfüzyonla alınarak Bouin fiksatifine kondu. Rutin histolojik takip işleminden sonra her gruptan ikişer testis Masson'un trikrom boyası ve orseinle boyanarak ışık mikroskobunda nitel olarak değerlendirildi. Altışar testis histomorfometrik inceleme için 1/8 oranında seri kesilerek PAS+hematoksilenle boyandı. Stereolojik inceleme için Cavalieri ve optik parçalama+optik disektör sondaları kullanıldı. Bulgular: Kontrol gruplarına kıyasla 1 ve 12 haftalık vazektomi gruplarında ortalama spermatogonyum, Sertoli ve miyoid hücre yoğunluğunun arttığı, spermatid sayısının ise azaldığı saptandı. Yirmi dört haftalık vazektomi grubunda ise seminifer tübülü oluşturan hücrelerin azaldığı görüldü. Nitel değerlendirmede vazektomi sonrası süreye paralel olarak ara maddedeki kolajen lif miktarında artış ve 1 haftalık deney grubunda seminifer tübül lümeninde dev hücre oluşumu gözlendi. Tartışma ve Sonuç: Vazektomi sonrasında 12. haftaya kadar seminifer tübül adlüminal ve bazal kompartımanlarında iki farklı mekanizmanın çalıştığı görülmüştür. İlkin, bazal kompartımanda gerek parakrin gerekse hücrelerarası bağlantılara bağlı bir telafi mekanizması çalışmaktadır. İkincisi, epididimiste gelişen seminal granülomu seminifer tübül iç basıncını artırarak adlüminal kompartımanda spermatid-Sertoli hücresi ilişkisini bozup spermatidlerin olgunlaşamadan hızla dejenere olmasına neden olabilir.
