A new conglomerate family sample of 194 dwellings with 996 resident persons were studied in the town of Barcelos, State of Amazonas, in order to re-evaluate the risk of Chagas disease. During the survey the persons were interviewed and in this occasion we showed to them a collection of Panstrongylus, Rhodnius and Triatoma, asking if they recognized and eventually have been bitten by this kind of bugs. At this time we collected 500 ul of blood in microtainer tubes from 886 interviewed persons who gave permission after informed consent. A screening test for T. cruzi antibodies based on agglutination of colored polymer particles, sensitized with three different synthetic peptides of T. cruzi (ID-PaGIA Chagas Test), showed 13.2% of sera positivity, but only 6.8% were confirmed by indirect immunofluorescence, and ELISA with purified T. cruzi antigens. Two hundred and six interviewed persons (20.7%) recognized the triatomines, as "piaçavas' lice" and 62 (30%) confirmed that have been bitten by the bugs, 25.8% of them had a positive serology for T. cruzi infection. Electrocardiographic alterations were shown in 9.3% of the seropositives and in 11.9% of the seronegative cases. This was considered not statistically significant.

A doença de Chagas tem como agente etiológico o Trypanossoma cruzi, um protozoário flagelado que pode ser encontrado numa grande variedade de mamíferos e triatomíneos. O Estado de Rondônia, localizado na Amazônia Ocidental, possui um meio ambiente constantemente modificado pelas ações transformadoras do ser humano, resultando em um desequilíbrio, que pode facilitar a transmissão de inúmeros patógenos. Uma grande variedade e quantidade de palmáceas, em especial o babaçu, bem como mamíferos e triatomíneos, podem ser encontrados neste complexo ecossistema. Nesta pesquisa, a fauna de triatomíneos foi identificada em 225 babaçus e por meio de capturas peri e intradomiciliares. Foi realizado, concomitantemente, estudo de soroprevalência para doença de Chagas e a identificação da presença de T.cruzi no trato digestivo dos triatomíneos. Positividade ao T. cruzi foi verificada em 23,7% dos 652 triatomíneos coletados nos babaçus. Estes triatomíneos pertenciam ao gênero Rhodnius e foram classificados em 4 espécies: R. robustus, R. prolixus, R. pictipes e R. milesi. Nas capturas intradomiciliares, dez espécimes do Rhodnius robustus e uma de Panstrongylus geniculattus foram encontrados, sendo que 3% da população foi positiva para doença de Chagas. Na área pesquisada, há potencial de transmissão da doença de Chagas na forma endêmica devido a grande quantidade de triatomíneos, bem como alta freqüência de infecção destes triatomíneos, porém no momento deste estudo não se evidenciou a ocorrência da transmissão.

Chagas disease control initiatives are yielding promising results. Molecular research has helped successful programs by identifying and characterizing introduced vector populations and by defining intervention targets accurately. However, researchers and health officials are facing new challenges throughout Latin America. Native vectors persistently reinfest insecticide-treated households, and sylvatic triatomines maintain disease transmission in humid forest regions (including Amazonia) without colonizing human dwellings. In these scenarios, fine-scale vector studies are essential to define epidemiological risk patterns and clarify the involvement of little-known triatomine taxa in disease transmission. These eco-epidemiological investigations, as well as the planning and monitoring of control interventions, rely by necessity on accurate taxonomic judgments. The problems of cryptic speciation and phenotypic plasticity illustrate this need -and how molecular systematics can provide the fitting answers. Molecular data analyses also illuminate basic aspects of vector evolution and adaptive trends. Here we review the applications of molecular markers (concentrating on allozymes and DNA sequencing) to the study of triatomines. We analyze the suitability, strengths and weaknesses of the various techniques for taxonomic, systematic and evolutionary investigations at different levels (populations, species, and higher taxonomic categories).Key words: molecular systematics, DNA, allozymes, Triatominae, Chagas disease. CHAGAS DISEASE: BURDEN, CONTROL EFFORTS AND NEW CHALLENGESChagas disease (CD), caused by Trypanosoma cruzi and transmitted by triatomine bugs, is a major public health problem in Latin America. An overall prevalence of ∼ 18 million infections was estimated in the late 1980s, with 90 million people living at risk and 45000 deaths/year. CD was the most important parasitic disease in Latin America in terms of its impact on national economies and public health systems (WHO 1991, World Bank 1993.In the absence of vaccines or adequate drugs for large-scale treatment, the reduction of disease burden critically depends on the control of transmission by triatomine vectors and infected blood transfusion. Several multinational initiatives have been launchedAn Acad Bras Cienc (2005) 77 (3) 438FERNANDO ABAD-FRANCH and FERNANDO A. MONTEIRO with that purpose. Ten years of concerted action in the Southern Cone have resulted in the elimination of transmission by Triatoma infestans (the most widespread domestic vector) from vast areas of the region. Incidence dropped by an average of 94% in the area, and by 65% in Latin America (WHO 2002).
