Aplastic crisis due to human parvovirus B19 infection in hereditary hemolytic anaemia

Abstract: Specific anti-B19 IgM was demonstrated in sera from three children showing transient aplastic crisis. A two years-old boy living in Rio de Janeiro suffering from sickle-cell anaemia showed the crisis during August, 1990. Two siblings living in Santa Maria, RS, developed aplastic crisis during May, 1991, when they were also diagnosed for hereditary spherocytosis. For a third child from this same family, who first developed aplastic crisis no IgM anti-B19 was detected in her sera.

“…Infection is widespread in Rio de Janeiro city where anti-B19 IgG antibodies were found in 73.7% of the sera collected during 1985 and 1986 25 . Clinical presentations such as TAC in hereditary spherocytosis and sickle cell patients 13 , fifth disease 12,26 , NIHF 11,16 were also described by us in Rio de Janeiro, and other Brazilian States during the last decade. We are reporting here a 20 months follow up, hold during 1994 to 1996, in a cohort of chronic hemolytic anemia patients attending at Instituto Estadual de Hematologia in Rio de Janeiro.…”

“…Although some studies (Freitas et al 1988, 1993, Miranda et al 1989, Nascimento et al 1990, Cubel et al 1992, Mielle et al 1995 have been published in recent years, including preliminary results of the first year of this study (Oliveira et al 1996), clinical and epidemiological aspects of parvovirus B19 are still not widely documented in Brazil. By systematically gathering data on acute rash diseases over the years we disclosed a seasonal pattern of the infection in a mid-sized urban area.…”

“…Infection of adults often manifests as arthropathy (26). In patients with chronic hemolytic anemias, such as sickle cell disease or hereditary spherocytosis, B19 infection causes transient aplastic crisis by destroying the erythroid precursor pool (11). Although these are the best-described clinical illnesses caused by B19, the virus has been implicated in a wide spectrum of other illnesses (25).…”
