Fatores perinatais relacionados com a morbidade e a mortalidade de recém-nascidos pertencentes a nove unidades neonatais do município de São Paulo

ResumoEm um total de 10235 nascimentos em 9 hospitais do Município de São Paulo, foram internados nas respectivas unidades de cuidados neonatais (UCN) 1975 recém-nascidos patológicos (RNP). Os objetivos foram avaliar características dos RNP, as condições mórbidas mais freqüentes, a prevalência do aleitamento materno, as terapêuticas utilizadas e os possíveis fatores de risco significantes para o óbito. Foi calculada a letalidade, a razão de chance com seu respectivo intervalo de confiança, foi realizado o teste de c 2 para avaliar a relação entre as variáveis de estudo e o óbito. A letalidade entre os RNP com baixo peso ao nascer, principalmente com peso <1500g, foi significantemente maior que a dos neonatos com peso > 2500g. A letalidade entre os prematuros e os pequenos para a idade gestacional foi também significativa quando comparada a outras categorias. Os RNP, com valores de Apgar < 7 no 1º e 5º minuto, tiveram prognóstico pior: a letalidade entre os RNP com essa característica foi, aproximadamente, dez vezes maior que a dos não expostos a essa situação. Quando a hipoxia foi prolongada, a letalidade foi o dobro da detectada entre os RNP com Apgar baixo apenas no 1º minuto. Somente 25,4% receberam leite materno exclusivo. As terapêuticas utilizadas foram as convencionais. Vários fatores de risco relacionados à mãe, ao parto e ao RNP foram importantes para a mortalidade, assim como a maioria das condições mórbi-das, refletindo a situação da assistência perinatal no nosso meio.

AbstractThis study was carried out in nine hospitals in the city of São Paulo, in 1991 (between July 1 and November 30), when 10235 live birth were born. The aim of this study was to analyze the characteristics of the 1975 pathologic newborn (PNB) who were put into those neonatal care units (NCU) of the hospitals; besides that, the most common morbidity conditions, breast feeding prevalent and therapies were studied. It was calculated the odds ratio, its confidence interval and the c 2 test in order to evaluate the association between the variables and the death of the PNB. The results have demonstrated that the low weight PNB letality (especially those with weight <1500g) was higher than those PNB whose weight was > 2500g. The letality for both premature PNB and small for gestational age PNB was higher than the other categories of babies. The PNB with hypoxia until the 5th minute had the worst prognostic: their letality was 10 times higher than the other category. The PNB who had Apgar <7 until the 5th minute had twice as much letality when compared with PNB who had Apgar <7 just until the 1th minute. Convention...
