Resumo O trabalho teve como objetivo identificar a percepção de gestores públicos da assistência farmacêutica em esferas subnacionais e de gerentes técnicos de instâncias colegiadas do Sistema Único de Saúde sobre os reflexos do Programa Farmácia Popular do Brasil (PFPB), iniciativa exclusivamente federal, na gestão da assistência farmacêutica na atenção básica (Afab), de responsabilidade tripartite e operacionalizada pelos municípios. Entrevistaram-se gestores municipais, estaduais e gerentes técnicos do Conselho Nacional de Secretarias Municipais de Saúde e do Conselho Nacional de Secretários de Saúde. A análise do conteúdo considerou as categorias (1) desafios e avanços da Afab e do PFPB e (2) conexões entre a Afab e o PFPB. Houve visões divergentes sobre as relações entre Afab e PFPB, se concorrentes ou complementares. Ressaltou-se o contraste entre os crescentes investimentos no PFPB e sua estagnação na Afab, a sobreposição dos elencos, a migração de usuários e o papel do PFPB como alternativa de acesso, entre outros. A implementação centralizada do PFPB parece ter se dado com baixa articulação com as esferas subnacionais de gestão, gerando distintas e contraditórias interpretações sobre seu papel e objetivos para os municípios, considerando-se as diretrizes de descentralização da assistência farmacêutica.

OBJECTIVE To analyze the costs of public pharmaceutical services compared to Farmácia Popular Program (Popular Pharmacy Program).METHODS Comparison between prices paid by Aqui Tem Farmácia Popular Program (Farmácia Popular is available here) with the full costs of medicine provision by the Municipal Health Department of Rio de Janeiro. The comparison comprised 25 medicines supplied by both the municipal pharmaceutical service and Aqui Tem Farmácia Popular Program. Calculating the cost per pharmaceutical unit of each medicine included expenditure by Municipal Health Department of Rio de Janeiro with procurement (price), logistics, and local dispensation. The reference price of medicines paid by Aqui Tem Farmácia Popular was taken from the Brazilian Ministry of Health standard in force in 2012. Comparisons included full reference price; reference price minus 10.0% copayment by users; and maximum reference paid by the Ministry of Health (minus copayment and taxes). Simulations were carried out of the differences between the costs of Municipal Health Department of Rio de Janeiro with the common medicines and those potentially incurred based on the reference price of Aqui Tem Farmácia Popular.RESULTS The Municipal Health Department of Rio de Janeiro spent R$28,526,526.57 with 25 medicines of the common list in 2012; 58.7% accounted for direct procurement costs. The estimated costs of the Health Department were generally lower than the reference prices of the Aqui Tem Farmácia Popular Program for 20 medicines, regardless of reference prices. The potential costs incurred by Health Department if expenditure of its consumption pattern were based on the reference prices of Aqui Tem Farmácia Popular would be R$124,170,777.76, considering the best scenario of payment by the Brazilian Ministry of Health (90.0% of the reference price, minus taxes).CONCLUSIONS The difference in costs between public provision by Municipal Health Department of Rio de Janeiro and Farmácia Popular Program indicates that some reference prices could be reviewed aiming at their reduction.

OBJECTIVE:To analyze resource allocation for pharmaceutical procurement by federative entities in the Brazilian Unifi ed Health System. METHODS:The amounts allocated to purchase pharmaceuticals during 2009 in two information systems were analyzed: Siga Brasil (Follow Brazil) for national data and Sistema de Informações sobre Orçamentos Públicos em Saúde (Information System on Public Health Budgets) for states, the Federal District and municipalities data. Per capita spending and the mean and median spending were calculated by municipalities, according to region and population size. The Spearman correlation coeffi cient was calculated for some variables. The statistical analysis included tests of normality and multiple comparisons for differences between groups. RESULTS:In 2009 the total amount spent by the three spheres of government for purchase of medicines was approximately R$ 8.9 billion. States and the Federal District were the main players, accounting for 47.1% of the total amount spent in the health system. Some states had per capita spending well above the mean (R$ 22.00 per resident/year) and the median (R$ 17.00 per resident/year). There were differences in municipal spending by region. The mean per capita expenditure of municipalities with less than 5,000 residents was 3.9 times that of municipalities with over 500,000 residents. Municipalities with less than 10,000 residents had higher per capita spending than other municipalities. CONCLUSIONS:Economic aspects such as the scale of procurement and bargaining power may explain differences in per capita spending between federal entities, especially among municipalities. The study indicates ineffi ciencies in the use of fi nancial resources to procure medicines in the Brazilian Unifi ed Health System.
