Introduction: The Kangaroo Mother Care (KMC) method is a significant neonatal alternative that ensures better quality humanized care for preterm and low birth weight newborns. Objective: To analyze the immediate physiological effects of the kangaroo position in critically ill newborns. Methods: Open clinical trial with parallel interventions, involving preterm (up to 28 days old) low or very low birth weight newborns (minimum weight of 1,250 grams) of both sexes, that were clinically stable and undergoing enteral nutrition. The degree of respiratory distress was assessed and quantified using the Silverman-Anderson scoring system. Heart rate and peripheral oxygen saturation were collected using a pulse oximeter. Respiratory rate was determined by auscultation for one minute. The newborns were submitted to the kangaroo position once only, for 90 minutes. Results: Participants were 30 newborns, 56.7% of which were girls. Comparison of the variables before and after application of the kangaroo position using the Wilcoxon test showed a statistically significant reduction in respiratory rate (p = 0.02) and Silverman-Anderson score (p < 0.01). The remaining variables showed no significant differences: heart rate (p = 0.21), peripheral oxygen saturation (p = 0.26) and axillary temperature (p = 0.12). Conclusion: There was a decline in the respiratory rate and Silverman-Anderson score after application of the kangaroo position, while peripheral oxygen saturation, axillary temperature and heart rate remained stable.

Objetivo: Comparar em recém-nascidos com taquipneia transitória se o método reequilíbrio tóraco-abdominal (RTA) aumentou a dor imediatamente após. Métodos: Estudo de ensaio clínico randomizado. Quarenta e nove recém-nascidos com diagnóstico de taquipneia transitória com menos de 72 horas de vida, foram incluídos para receber fisioterapia respiratória. Os participantes receberam os cuidados usuais e uma sessão de fisioterapia convencional ou do método reequilíbrio tóraco-abdominal. Foram registradas a escala NIPS (Neonatal Infant Pain Scale), a saturação periférica de oxigênio, a frequência cardíaca, a frequência respiratória e a temperatura axilar antes e depois da fisioterapia. Para as comparações entre as medidas, foram utilizados o teste de ANOVA de Kruskal-Wallis e o teste de McNemar. O risco relativo de dor após os procedimentos foi calculado usando o modelo de regressão de Poisson (estimação robusta). Foi considerado o nível de significância de 5% para todas as análises (p < 0,05). Resultados: O método RTA não foi associado a dor. Após a fisioterapia respiratória, a escala NIPS reduziu (2 versus 3, p < 0,001) e a proporção de recém-nascidos com dor também reduziu (10,2% versus 28,6%, p = 0,02). O risco relativo de dor após a fisioterapia respiratória em comparação a antes, foi de 0,3 (IC 95% 0,15-0,41; p = 0,02), a frequência respiratória diminuiu (58 versus 70, p < 0,001) e a saturação periférica de oxigênio aumentou (98% versus 96%, p < 0,001). Conclusão: Em recém-nascidos com taquipneia transitória nas primeiras 72 horas de vida, o método RTA não influenciou a avaliação da dor, a fisioterapia respiratória foi segura e reduziu a dor imediatamente após.

Introdução: Segundo as orientações do Ministério da Saúde e da Organização Mundial da Saúde, a gestante deve optar pelo parto transpélvico, caso não tenha nenhuma indicação de cesárea. Apesar disso, Brasil é um dos países que mais realiza cesárea no mundo todo, sendo que apenas 15% são realmente necessárias. Objetivo: Realizar um estudo comparativo em relação ao estado clínico da parturiente e do recém-nascido após a realização de parto transpélvico e cesárea. Material e Métodos: Foram coletados dados de 50 gestantes com idade gestacional entre 38 a 40 semanas quando realizaram o parto, através da análise de prontuário e questionário aplicado às puérperas, e contemplando o estado de saúde tanto da puérpera como do recém-nascido. Resultados: Com as informações levantadas, foi traçado um perfil das gestantes. O estudo estatístico não demonstrou significado estatístico na comparação das vias de parto com relação aos seguintes aspectos: ocorrência e intensidade da dor após o parto (P = 0,14), Apgar do 1º minuto (P = 0,73) e Apgar do 5º minuto (P = 0,53). Apenas o tempo de permanência mostrou-se significativo (P = 0,02), sendo encontrado um maior tempo de permanência do parto transpélvico. Conclusão: Apesar de a literatura demonstrar maiores vantagens tanto para a paciente como para o recém-nascido quando realizado o parto transpélvico, o presente estudo não encontrou diferenças significativas entre as duas vias de parto.Palavras Chave: Parto transpélvico, Cesárea, Período pós-parto, Recém-nascido, Estudo comparativo ABSTRACTIntroduction: According to the guidelines of the Ministry of Health and the World Health Organization, pregnant woman should opt for transpelvic delivery, if she has no indication for cesarean section. Nonetheless, Brazil is one of the most performing cesarean section countries in the world, but only 15% of the cases the method are really needed. Objective: To perform a comparative study regarding the clinical status of parturient and newborn after transpelvic and cesarean delivery. Material and Methods: Data were collected from 50 pregnant women with gestational age between 38 and 40 weeks when they delivered, through analysis of medical records and questionnaire applied to postpartum women and considering the health status of both the postpartum and the newborn. Results: With the information gathered, a profile of the pregnant woman was drawn. The statistical study did not show statistical significance in the comparison of the pathways regarding the following aspects: occurrence and intensity of pain after delivery (P = 0.14), 1st minute Apgar score (P = 0.73) and 5th minute Apgar score (P = 0.53). Only the length of stay was significant (P = 0.02), and a longer time of transpelvic delivery was found. Conclusion: Although the literature demonstrates greater advantages for both the patient and the newborn when performing transpelvic delivery, the present study found no significant differences between the two routes of delivery.Keywords: Transpelvic delivery; Cesarean section; Postpartum period; Infant, newborn; Comparative study
