Clinical outcome of neonatal bacterial meningitis according to birth weight

Abstract: -Objective: To describe the clinical outcome and the complications of bacterial meningitis according to birth weight of out born neonates admitted in intensive care unit during an 11 year-period. Method: Eighty-seven newborns were studied. Thirty-four infants were low birth weight newborn and 53 presented birth weight ≥2500 g. The clinical data were obtained through the analysis of patients' files. Fisher's exact test, the χ 2 and the Mann-Whitney test were applied. Results: Neurological symptoms were more com… Show more

“…En cambio, para la tardía se asocian factores relacionados con terapia intensiva como procedimientos invasivos y técnicas neuroquirúrgicas (2,3,8,9) . La frecuencia de aislamiento del patógeno es variable, 14% hasta 78%, y usualmente se describen Streptococcus del grupo B (GBS), Escherichia coli y Listeria monocytogenes (2,3,(10)(11)(12) .…”

“…The authors pointed out that meningitis frequently occurs in the absence of bacteremia and, for that reason, lumbar puncture is an important part of the diagnostic investigation in cases of suspected sepsis, since it may be the only positive test 11 . Another study conducted in São Paulo on 87 newborns with meningitis diagnosed by cytological and biochemical alterations reported that 39% of the cases had positive CSF cultures and only 17% showed positive blood cultures 24 . Hematological tests have not been shown to be useful for differentiating between the agents and are unspecific in relation to infection and meningitis diagnosis 22 .…”

“…In agreement with data in the literature, complications such as ventriculitis, hydrocephaly and abscesses were more frequent in Gram-negative meningitis. Ventriculitis may occur in 40 to 90% of the cases, particularly when bacteria persist in the CSF, while hydrocephaly is usually associated with ventriculitis and hemorrhage 24 . The frequency of abscesses was rather high (15%) in comparison with the rates of 1% to 3.4% reported in the literature 24,26 .…”

“…(Volpe, 2008a(Volpe, ,2008b. Inefficiency in the neonates alternative complement pathway compromises their defense against encapsulated bacteria (Krebs et al, 2007) T-cell defense and mediation of B-cell activity also are compromised. Finally, deficient migration and phagocytosis by neutrophils contribute to neonatal vulnerability to pathogens of even low virulence.…”
