Severe hyponatremia and the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) associated with fluoxetine: case report

Abstract: -Hyponatremia is a significant complication of treatment with serotonin selective re u p t a k e inhibitors (SSRI). We describe a case of a 53-year-old woman that was started on fluoxetine 20 mg/day for d e p ression. Nine days later, the patient started with weakness, nausea, pro g ressing to confusion, inapetence and vomit. Three hours later she became unresponsive and had a generalized seizure. She was b rought to our emergency service. On admission, the patient was normovolemic, without focal motor deficit… Show more

“…Diuretics, carbamazepine, oxcarbazepine, fluoxetine, and other selective serotonin reuptake inhibitors have been associated with SIADH or enhanced renal responsiveness to ADH and subsequent low serum sodium levels [18][19][20]. Thiazide diuretics are common causes of hyponatremia that can result in either hypoor euvolemic fluid states [21].…”

“…Hyponatremia is the most common electrolyte abnormality observed in a general hospital population and it should be especially considered in patients taking a selective serotonin re-uptake inhibitor 2,3 . First described in 1959 by Adams et al 1 as central pontine myelinolysis and now more appropriately described as osmotic demyelination syndrome, this condition is characterized by regions of demyelination throughout the brain most notably when hyponatremia had been corrected rapidly.…”
