Cerebrovascular disease in neonates: evaluation of four cases

Abstract: -The clinical and neurological study in four neonates infants with cerebral infarction are reported. The purpose of this study is to call attention for the clinical course, cranial ultrasound, computed tomography and laboratories tests, in order to evaluate the neurological sequelae. A careful evaluation has be taken in order to determine the significance of clinical and laboratory tests for syndromic, topographic and etiologic diagnosis after one year ambulatorial follow-up.KEY WORDS: neonatal stroke, perinat… Show more

“…Balcom and Redmond 6 , in 1997, estimated CVD in term newborns as being 1/ 10,000, while in the nursery of the Division of Pediatric Neurology, Campinas State University School of Medical Sciences, the incidence is 2.8 / 10,000 term newborns. These data confirm the importance of recognizing CVD in neonates 7 . The neurological and imaging studies in the present investigation demonstrated the presence of hemorrhage in two newborns (Cases 1 and 2) and ischemia evolving to hemorrhage in one (Case 3).…”

“…However, brain SPECT imaging performed at 4 years of age revealed a persistent hypoperfusion in the left fronto-temporal region and in the left cerebellar hemisphere (ipsilateral cerebellar diaschisis). Diaschisis is defined as reduction of the blood flow and neuronal metabolism in a remote area from the primary cerebral lesion, which leads to failure of the facilitation mechanisms, as a consequence of a destructive lesion in the interconnecting pathways 1,[7][8][9] . In supratentorial lesions, this abnormality can be frequently detected in the contralateral cerebellar hemisphere due to destruction of the cortico-pontocerebellar pathways.…”

“…The ischemic type was the most common, and involvement of the middle cerebral artery, sudden onset of clinical manifestation with seizures and motor disability were more common in early aged children. Motor sequelae predominated in the follow-up of these children.KEY WORDS: cerebrovascular disease, stroke, children.

“…We did not find newborn infants in the present series, probably due to difficulties in performing imaging exams during the acute phase of the disease at this time during the investigation. In newborn infants, when peri-intraventricular hemorrhages are excluded, cerebrovascular disease more frequently presents convulsive seizures as the initial manifestation 8,9 .…”
