-Objective: To compared the clinical features of 373 patients with primary and postparalytic hemifacial spasm (HFS). Method: Data analyzed were gender, ethnicity, age at symptom onset, disease duration, affected side, distribution of facial spasm at onset, hypertension, family history of HFS, previous history of facial palsy and latency between facial palsy and HFS. Results: The prevalence of patients with Asian origin was similar in both groups such as female/male ratio, mean age at symptom onset, disease duration, affected side and distribution at onset of facial twitching. The upper left side of the face was the main affected region at onset. Almost 40% of the patients in both groups had hypertension. A prevalence of vascular abnormalities on the posterior fossa was seen in 7% and 12.5% of both groups. Conclusion:The clinical profile and radiological findings of patients with primary and postparalytic HFS are similar. The association of hypertension with vascular abnormalities and HFS was not frequent.KEY WORDS: hemifacial spasm, facial palsy, neuroimaging, neurovascular contact, hypertension.análise clínica de pacientes com espasmo hemifacial primário e pós-paralítico: estudo retrospectivo RESUMO -Objetivo: Comparar características clínicas de 373 pacientes com espasmo hemifacial (EHF) primário e pós-paralítico. Método: Os dados analisados foram: sexo, etnia, idade no início dos sintomas, duração da doença, lado afetado, distribuição dos espasmos no início da doença, hipertensão arterial, histó-ria familiar de EHF, história prévia de paralisia facial periférica e latência entre a paralisia facial e o início do EHF. Resultados: A prevalência de pacientes com origem asiática foi semelhante em ambos os grupos assim como razão homem/mulher, média de idade no início dos sintomas, duração da doença, lado afetado e distribuição dos espasmos no início dos sintomas. O quadrante superior esquerdo da face foi o lado mais afetado no início dos sintomas. Quase 40% dos pacientes em ambos os grupos tinha hipertensão arterial. A prevalência de anormalidades vasculares na fossa posterior foi observada, respectivamente, em 7% e 12,5% dos casos. Conclusão: O perfil clínico e também radiológico dos pacientes com EHF primá-rio e pós-paralítico foi semelhante. A associação entre hipertensão arterial e anormalidades vasculares na fossa posterior não foi freqüente. ObjectiveThe purpose of this study was to evaluate the characteristics and surgical outcomes of familial hemifacial spasm (HFS) and to discuss the role of genetic susceptibility.MethodsBetween 2001 and 2011, 20 familial HFS patients with ten different pedigrees visited our hospital. The data from comprehensive evaluation of these patients, including clinical, radiological and electrophysiological data and surgical outcomes were reviewed to characterize familial HFS and to compare the characteristics between familial HFS and sporadic HFS.ResultsAccording to the family tree, the inheritance pattern was difficult to define clearly using these data. Radiologic findings suggested that the vertebral artery (VA) was a more frequent offender in familial HFS than in sporadic cases (35.0% vs. 10.0%, p<0.001). Chi-square test showed that there were no correlation between VA tortuosity and underlying morbidity such as diabetes or hypertension (p=0.391). Eighteen out of 19 patients who underwent microvascular decompression showed no residual spasm. Other features of familial HFS overlap with sporadic cases. These findings suggest that certain genetic susceptibilities rather than hypertension or diabetes may influence vascular tortuosity and HFS development.ConclusionIn this study, familial HFS seems not so different from sporadic cases. Authors thought familial HFS could have heterogeneous etiology. Further study of familial HFS including clinical, anatomic, genetic, and molecular information may help identify a gene or trait that can provide insight into the mechanisms of sporadic and familial HFS.

Os casos refratários tornam-se um desafio para o cirurgião plástico. Relato do Caso: MSP, sexo feminino, 53 anos, portadora de EH, refratária à DMV, e com queixas de flacidez dos dois terços inferiores da face. Foi submetida à ritidoplastia, neurectomias e miectomias seletivas. Obteve-se ausência completa dos espasmos, manutenção da mímica facial e rejuvenescimento facial. A ritidoplastia cérvico-facial associada a neurectomias e miectomias mostrou-se eficaz no tratamento do EH.Descritores: Face/cirurgia. Espasmo hemifacial. Espasmo. Ritidoplastia. ABSTRACT Introduction:The peripheral facial palsy is a disease that can leave significant aesthetical and functional sequelae in patients. According to medical literature, Bell's palsy (BP) is the most common cause, representing from 50 to 80% of the cases. However, it must be considered as an exclusion diagnosis. A slightly higher prevalence is estimated in women and its incidence is bimodal with peaks in the third and eighth life decades. Most patients recover within weeks, though a worsening degree may be common in the first 48 hours. Treatment must be performed to prevent complications and the medical procedures are not universally accepted. The following have been mentioned as BP risk factors: arterial hypertension, mellitus diabetes, pregnancy, puerperium and infection caused by type I herpes virus. Although there are several BP associated risk factors, regarding face-lifting surgery there is only one reported case in medical literature. Case Report: In the present study, the authors describe the second case of Bell's palsy after face-lifting surgery.
