Watermelon stomach: clinical aspects and treatment with argon plasma coagulation

Abstract: There seem to be two distinct groups of patients with gastric antral vascular ectasia: one related to immunologic disorders and other to non-immune chronic liver disease and low platelets. The endoscopic treatment using argon plasma coagulation had a high recurrence in the long-term evaluation.

“…Cirrhosis is commonly associated with a diffuse appearance of GAVE, involving the whole antrum without interruption; whereas non-cirrhotic patients are more likely to have a typical striped "watermelon" appearance. 11,12 Our two patients fit these respective endoscopic patterns. Epidemiological differences have been noted in these two groups, and characteristics of the two patterns are listed in table 1.…”

“…As an important diagnostic method, the experience of our patient 2 told us that repeated endoscopy is necessary when facing unexplained chronic blood loss patient. Here we summarized typical endoscopic performance of GAVE into two distinctive patterns: The majority of patients including our two cases have antral disease with classic ectatic vascular tissue radiating out from the pylorus that described by Jabbari et al as WS; another type has scattered red patchy or punctuate lesions distributed diffusely throughout the proximal and distal stomach and its predilection sites also located in the antrum [11]. GAVE patients with cirrhotic syndrome tend to have diffuse le-sions under endoscopy and most of them are males (75%) with mean age of 65 years old.…”

“…2 A maioria dos casos é idiopática, porém é frequentemente associada a outras condições clínicas, incluindo cirrose, esclerose sistêmica, e doença cardiovascular. 4 A base terapêutica da GAVE é a ablação endoscópica estudos tem avaliado a utilização das técnicas termoablativas. 4 Normalmente, várias sessões são necessárias para alcançar o controle do sangramento agudo e para corrigir a anemia Uma das terapias endoscópicas é o laser de coagulação Nd:…”
