AimsTo determine a normative of tumbling E optotype and its feasibility for visual acuity (VA) assessment in children aged 3-4 years.MethodsA cross-sectional study of 1756 children who were invited to participate in a comprehensive non-invasive eye exam. Uncorrected monocular VA with crowded tumbling E with a comprehensive ophthalmological examination were assessed. Testability rates of the whole population and VA of the healthy children for different age subgroups, gender, school type and the order of testing in which the ophthalmological examination was performed were evaluated.ResultsThe overall testability rate was 95% (92% and 98% for children aged 3 and 4 years, respectively). The mean VA of the first-day assessment (first-VA) and best-VA over 2 days’ assessments was 0.14 logMAR (95% CI 0.14 to 0.15) (decimal=0.72, 95% CI 0.71 to 0.73) and 0.13 logMAR (95% CI 0.13 to 0.14) (decimal=0.74, 95% CI 0.73 to 0.74). Analysis with age showed differences between groups in first-VA (F(3,1146)=10.0; p<0.001; η2=0.026) and best-VA (F(3,1155)=8.8; p<0.001; η2=0.022). Our normative was very highly correlated with previous reported HOTV-Amblyopia-Treatment-Study (HOTV-ATS) (first-VA, r=0.97; best-VA, r=0.99), with 0.8 to 0.7 lines consistent overestimation for HOTV-ATS as described in literature. Overall false-positive referral was 1.3%, being specially low regarding anisometropias of ≥2 logMAR lines (0.17%). Interocular difference ≥1 line VA logMAR was not associated with age (p=0.195).ConclusionsThis is the first normative for European Caucasian children with single crowded tumbling E in healthy eyes and the largest study comparing 3 and 4 years old testability. Testability rates are higher than found in literature with other optotypes, especially in children aged 3 years, where we found 5%–11% better testability rates.

OBJECTIVES:To describe the indications for and visual outcomes of intrastromal corneal ring segment implantation.METHODS:A large retrospective case-series chart-review study was conducted using Sorocaba Ophthalmological Hospital medical records. This study included 1222 eyes (1196 patients) that were surgically treated between November 2009 and December 2012. The following preoperative data were collected: age, gender, type of medical care and funding source, surgical technique, best-corrected visual acuity, manifest sphere and cylinder refractive error, maximum and minimum central keratometry, and pachymetry measurements of the cornea at the thinnest point and at the ring channel. The postoperative best-corrected visual acuity and patient satisfaction were also determined. The cases were classified into six groups: four keratoconus groups (severe, advanced, moderate and mild), a pellucid marginal degeneration group and a post-graft irregular astigmatism group. This study was approved by the Brazilian Registry of Clinical Trials (UTN number 1111-1182-6181, TRIAL RBR-6S72RF).RESULTS:The age (mean±standard deviation) of the patients was 31.0±10.0 years. The most prevalent pathology was keratoconus (1147 eyes, 93.8%). A correlation was found between ectasia severity and medical assistance (p<0.001), and the most serious cases was treated by the Brazilian public health system. No complications were found in a total of 1155 surgeries, and after surgery, 959 patients were satisfied. Among the 164 dissatisfied patients, the majority failed to show improved best-corrected visual acuity.CONCLUSION:Patients in the public health system underwent surgical intervention for keratoconus later than those with private sources of funding. In the vast majority of operated cases, the patients reported improvements in vision.

RESUMO: o objetivo foi comparar a frequência das ações motoras em cubos com estímulos visuais (luminoso e alto-contraste) e sem estímulos visuais (transparente e preto) em crianças com baixa visão e visão normal. Seis crianças com baixa visão (43 meses; ±2) e sete crianças com visão normal (42,3 meses; ±2,9) participaram. Os materiais utilizados foram quatro cubos (luminoso, alto- contraste, transparente e preto) e duas câmeras filmadoras. Todas as avaliações foram filmadas e analisadas para aferir a frequência das ações motoras. Cada cubo foi apresentado à criança por um minuto, com intervalo de 15 segundos entre eles e a sequencia de entrega dos cubos foi randomizada. Ocorreram diferenças significativas na frequência das ações motoras entre as crianças com baixa visão e visão normal para o cubo de alto-contraste (p=0,036), sendo que as com baixa visão apresentaram maior frequência em relação as com visão normal. No cubo de alto-contraste também foram encontradas diferenças significativas na frequência de alcance bimanual (p=0,027) e o girar o cubo (p=0,006), sendo maior no grupo baixa visão. O cubo com alto contraste estimulou as crianças com baixa visão a realizarem mais ações motoras, em especial o alcance bimanual e o girar o cubo.
