Temporomandibular disorders (TMD) are a set of characteristic clinical conditions of pain or discomfort of nondental origin, in the masticatory muscles, temporomandibular joint (TMJ) and associated structures. The main symptoms of these disorders are: cracking in the joints, difficulty in opening and closing the mouth, and pain. Therapies for treating TMD involve dietary and behavioral changes, pharmacotherapy, physical therapy, occlusal splint therapy, lasers or open joint surgery. In particular, the use of low-level laser (LLLT) is a non-invasive and non-pharmacological treatment that has shown beneficial results in the treatment of TMD-related pain. It may act through several mechanisms of action, including facilitating the release of endogenous opioids, increasing tissue repair and cellular respiration, increasing vasodilation and pain threshold, and decreasing inflammation. OBJECTIVE: To carry out a bibliographic survey on the efficacy of using LLLT in the treatment of TMD. METHODS: A narrative review was carried out, whose searches dated from November 2021 and in the databases: PubMed, Scielo, Llilacs and BBO. The descriptors "photobiomodulation therapy" and "temporomandibular disorders" and their corresponding ones in English were used. Articles published from 2011 to 2021, in Portuguese, English and Spanish, were included, prioritizing randomized clinical trials and systematic reviews. RESULTS: After reading the titles and abstracts of the 130 articles, 63 were selected for full reading. In the end, 18 were selected. There was positive corroboration among 11 of the authors regarding the effectiveness of low-power laser in improving chronic TMD pain. Its use offers advantages, as the therapeutic regimen is noninvasive, reversible, with fewer adverse effects. Despite this, there is still no consensus on the protocols to be used. Therefore, it is necessary to analyze each case and individualize the treatment in relation to the laser parameters, the number and duration of applications and the characteristics of the light beam. CONCLUSION: The evidence showed that the use of LLLT was effective in reducing symptoms resulting from TMD.

A low-power laser is presented as a treatment proposal for painful Temporomandibular Disorders (TMD). This randomized clinical trial evaluated the effectiveness of laserpuncture using low power laser in the treatment of pain in patients with temporomandibular disorders with and without concomitant administration of vitamin B12 (methylcobalamin). A questionnaire was applied to the participants to assess the location of the orofacial pain in conjunction with the request for blood vitamin B12 dosage. The groups were randomly divided, being Group 1: placebo laserpuncture and placebo vitamin B12; Group 2: effective laserpuncture and placebo vitamin B12, and Group 3: effective laserpuncture and effective vitamin B12. The study consisted of 45 adult patients (over 18 years old), regardless of sex and race, with signs and symptoms of TMD, where pain was the main complaint. In all sessions, pain was assessed using a visual analogue scale, before and after laser therapy. Laserpuncture was applied to points of Traditional Chinese Medicine, used for treating Temporomandibular Disorders, using a laser device with an infrared wavelength and energy of 3J. Eight sessions were heldand two sessions per week for 4 weeks. Groups 2 and 3 showed improvement in pain symptoms after treatment. Group 3 patients in the fourth session already showed the effective result of the treatment. Laserpuncture when associated with the use of vitamin B12 becomes more efficient for treating TMD.

Objetivo: realizar uma revisão sistemática sobre o uso da Laserterapia nos casos de Disfunção Temporomandibular. método: Foi realizada uma revisão de literatura, que utilizou artigos das bases de dados PubMed, Lilacs e Scielo, que tinham como objetivo a atuação fonoaudiológica com Laserterapia nos casos de DTM, publicados em português, entre os anos de 2016 e 2022 com os descritores “laserterapia”, “disfunção temporomandibular” e “odontologia”. Foram excluídos os artigos publicados em período inferior ao pré-estabelecido, que realizavam a Laserterapia no tratamento de outras patologias e que não descreveram o uso da Laserterapia como recurso para o tratamento da DTM. resultados: Após a triagem, 12 artigos foram selecionados, nos quais os resultados apontaram que o LASER é eficaz no controle da disfunção temporomandibular dolorosa, possibilitando redução da intensidade de dor e da movimentação mandibular, podendo-se observar também melhora da qualidade de vida desses indivíduos. conclusão: Conclui-se que a utilização da Laserterapia é considerada eficaz para o tratamento dos casos de DTM, já que por meio dos estudos apresentados nesse trabalho, pôde-se concluir que quando associada à terapia miofuncional, esse recurso traz diversos benefícios para o indivíduo, como diminuição da dor, melhora da abertura de boca, equilíbrio das funções do sistema estomatognático e melhora da qualidade de vida.
