Expenses of the Brazilian Public Healthcare System with chronic kidney disease

Abstract: Renal diseases and some of the main related diseases corresponded to 12.97% of the expenses in the triennium 2013-2015, and RRT to more than 5% of the SUS expenses with medium and high complexity healthcare. Such high expenses determine great concerns on the future maintenance of treatment for stage-5 CKD in Brazil and countries in similar or worse conditions of development.

“…14 In Brazil, from 2013 to 2015, there was an increase of 361.32% and 364.84%, respectively, in the costs with HD in patients with HIV and/or hepatitis B and/or C. Moreover, HD performed in three weekly sessions amounted 95.96% of procedures and 96.07% of spendings. 25 These results are similar to the values found in the Northeast region, in which the HD in the cited modality corresponded to 97.9% ( Table 2) and accounted for 93.6% of costs (table 3).…”

“…According to the authors, such results may be due to the increased number of people with such conditions that survived the initial phase of the disease because of technological and therapeutic advance or even the increased incidence and diagnosis of HIV and hepatitis in the population. 25 The spendings with the treatment of CRF tend to increase due to the population aging and incidence of NCCD, especially hypertension and DM. 25,26 In 2003, the total public spending in Brazil with renal replacement therapies were R$ 17,414.742,205,28, which correspond to approximately 8% of resources.…”

“…Costs were charged at R $ 57,892,010.54 by serum creatinine test; at R $ 1,801,647.28 for creatinine clearance; at R $ 49,998,226.18 per serum urea; R $ 32,148.24 for renal and urinary tract ultrasound; R $ 1,886,073.27 per 24-hour proteinuria, among other costs. 23 Thus, the high values of financial resources for the treatment of CKD in Brazil are found. In this perspective, it is warned that the disease presents an increasing behavior annually, due to the higher survival rate and, consequently, the emergence of non-communicable chronic diseases, such as systemic arterial hypertension and Diabetes Mellitus.…”

“…Chronic Kidney Disease (CKD) is characterized as a public health problem, whose prevalence is alarmingly increasing worldwide, and represents a difficulty in completing the physical, mental and social well-being states in addition to a need for healthcare, reflecting the rapid and temporary loss of kidney function (3) .…”
